25 | Licensing Program Analysts (LPAs) Amanda Blesi and Lea Habtom arrived to the facility at 9:30 AM on Wednesday, January 4, 2023 for a case management visit to clear deficiencies that were cited on December 20, 2022. Upon arrival LPAs observed 6 toddlers supervised by staff #1 and staff #2. There were 6 infants in the napping area supervised by staff #3 and staff #4. During today's inspection LPAs observed child #1 swaddled while sleeping in a crib. This is a violation of Safe Sleep Regulations and a Type A deficiency was issued during the inspection.
LPAs observed the facility was in ratio and using qualified staff today and the Type A deficiencies cited on December 20, 2022 are cleared during today's inspection. Notice of clearance letter provided to Paolo.
LPAs observed the Notice of Site Visit from inspection dated 12/20/22 was not posted at the facility. Civil Penalties are assessed in the amount of $100. See LIC 421 CC.
Title 22 Deficiency has been cited on the attached LIC 809-D. LPA Amanda informed facility representative Paolo Sarmiento that this report dated 1.04.2023 document(s) one Type A citation(s) which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care. Also, LPA Amanda Blesi informed the licensee Paolo Sarmiento to provide a copy of this licensing report dated 1/04.2023 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
Exit interview conducted and report was reviewed with facility representative Paolo Sarmiento. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. |
