An unannounced inspection was conducted today by Licensing Program Analyst Owens. LPA Owens met with Owner, Bonnie Asadi Lee and Director Brandi Higgins.
The purpose of the inspection is to close a complaint investigation that was originally opened on December 12, 2021. Interviews were conducted and a census was taken.
Based on interviews, documents and photos the facility did not sanitize a toy one child had in it's mouth before given the same toy to another child. The allegation staff did not sanitize toys is substantiated.
Based on interviews, documents and photos the facility did have a regular size bed pillow in a crib with a child asleep on the pillow. The allegation staff did not practice safe sleep procedure is substantiated.
Based on interviews, observation and photos the facility did have a baby lounger that has been recalled in the infant baby breaths classroom and in use. The allegation the facility is providing unsafe equipment is substantiated.
The preponderance of evidence standard has been met during this investigation, therefore the above allegations are found to be SUBSTANTIATED. Violations of the California Code of Regulations, Title 22, Division 12 & Chapter 3 are being cited on the attached LIC9099D.

These are Type A deficiencies, hence AB633 Notification Applies: Upon receipt of this report, the report must be posted along with the notice of site visit for 30 days for parents to view. Licensee must inform the parents/guardians of children in care at the facility and to the parents/guardians of children newly enrolled at the facility during the next 12 months via form LIC 9224 Acknowledgement of Receipt of Licensing Reports.

Infant Care General Sanitation:
Objects used by infants that are mouthed, shall be washed and disinfected at least daily, or more often if necessary. Such objects shall include, but not be limited to, toys and blankets.

Owner stated she has changed procedure. Each child has their own bucket of toys to play with during the day and the toys are sanitized at the end of each day.

This requirement is not met as evidence by based on photos an infant was playing with a cloth toy and had it in its mouth and then given to another infant to play with within minutes before being sanitized. This is an immediate risk to a child.

Owner stated she will remind staff at all staff meeting that pillows shall not be used in the crib. The large size pillow was removed from infant baby breathe classroom at time of inspection.

Based on conflicting interviews, the allegation that the facility doesn't follow infant's feeding plan is unsubstantiated. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the allegation did or did not occur.

An exit interview was conducted. Appeal rights were emailed and explained to the licensee.
