**This is an amended report to add the toddler option**

Due to COVID-19 LPA Habtom was unable to complete the capacity increase based on normal practice of an unannounced inspection. LPA Habtom conducted a televisit using facetime on July 28, 2020 at 2:10 pm with the administrator Michelle Zemlicka.

Licensing Program Analysts (LPAs) Lea Habtom met with Director Michelle Zemlicka for the purpose of a case management inspection. An application was received for a request to increase capacity from 7 infants to 19 serving 12 infants with a toddler component of 7. LPA visually toured the facility using facetime inside and out. The census upon arrival was 5 infants being supervised and cared for by 2 teachers. All adults present during today’s inspection have obtained a fingerprint clearance with the department. The infant program operates in 2 classrooms, infant room and toddler room.

Indoor Activity Space
Due to COVID-19, licensee provided measurements for the new indoor infant activity space. The new indoor infant activity area measured a total of 425 square feet which will accommodate the requested capacity of 12 infants. Measurements for the previous infant play room, now toddler room, was used from the report dated 11/29/19 of 263.2654 square feet which accommodates licensee’s request for 7 toddlers. The combination of the two rooms measurements accommodates licensee’s request to increase the capacity to 19.

Infant room:
A total of 1 sink was observed to be operable in the infant room.

The infant changing table has a padded vinyl surface not less than one-inch thick and raised sides at least three inches high and a sink is within arm’s reach.


A small kitchen area contains a refrigerator where bottles and food will be stored for infants. Cleaning compounds are kept in a cabinet out the reach of children. Water is made accessible by means of a water fountain or sippy cups.
Facility has accommodations for infant’s personal storage. Facility is equipped with sufficient age appropriate furnishings and equipment. A total of 4 cribs were observed to be available for infants. Licensee is aware that she may only serve infants under 12 months based on the number of cribs she has available. A fire clearance had been received from the Auburn City Fire Department for a capacity of 19.
Toddler room:
LPA observed a sufficient amount of equipment, toys, tables, chairs, high chairs, cubbies and napping mats. LPA observed cleaning disinfectants are appropriately stored and inaccessible to children. LPA observed a changing table within arm's reach of a sink. LPA observed sippy cups for water in the classroom. Measurements from previous visit dated 11/29/19 were used. The total classroom space contains a total of 263.2654 square feet, which will accommodate licensee’s request of 7 toddlers ages 18-36. There are three toilets and four sinks for the children, and a separate private restroom for the staff.

Outdoor Activity Space
There is one infant outdoor area, courtyard, on the property. The outdoor play area is fenced that is at least four feet tall. LPA observed a sufficient amount of equipment and toys. There are no bodies of water on the premises. There are shaded areas supplied by trees and overhangs. Measurements for the outdoor space was used from visit dated 11/29/19. The outdoor play area contains a total of 2155.125 square feet, which accommodates licensee’s request for 12 infants and 7 toddlers. Licensee acknowledges infants and toddlers may not use the space at the same time.

No deficiencies cited during today’s inspection. Notice of Site visit posted.
Effective 7/31/2020, the license is granted for 19, 12 infants and 7 toddlers. Licensee acknowledges that she may not exceed the capacity limitation of 19 on the license.
