13 | Licensing Program Analyst (LPA) Christopher Bello arrived at the facility at approximately 12:10pm and met with Site Supervisor Staci Thatcher to open and close a complaint investigation, regarding the above allegation. Upon arrival, LPAs observed 26 children with four teachers. LPA made observations, obtained documents pertaining to the investigation and conducted interviews. It was alleged that the facility did not provide a parent with their child’s school documents upon request. Interviews and documents corroborated the allegation. This is considered as a potential risk to the children in care. Staci stated that they did not deny the parent but wanted to confirm the parent’s identity before releasing the documents to them. Based on interviews and information gathered, the preponderance of evidence was met and the above allegation is found to be Substantiated. As a result of the findings, A Technical Advisory note, LIS9102-Technical Violation was issued. Appeals Rights were given to the director at time of inspection. A Notice of site visit given and must be posted for 30 days.
There were no Title 22 deficiencies during today’s investigation. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Director.
