front near the toddler entrance. Infant children will nap in cribs. Families will provide AM/PM snack and a lunch daily and will not be prepared on site. Bottles are provided by child's authorized representative and will be washed and sanitized with a 3-step process in between each use. Bottles will be labeled and stored separately in the refrigerator in the classroom. . Needs and Services plan will be updated every quarter. For infants under the age of 12 months, the LIC 9227 form is required to have on file.
Age appropriate toys and activities are available for children. There is a waiver on file dated 7/30/21 for the infants to share the play yard with the toddler component and each program will use the yard at separate times. There is grass and wood chip cushioning material underneath the play structures sufficient to absorb falls. There are large trees present to provide shade in the play yard. There are no bodies of water located on the property and none are to be added without prior notification and approval of the licensing agency. Any proposed changes to the physical plant, telephone number, or change of address shall be immediately reported to the Department.
Notice of Site Visit shall be posted for 30 days from today's visit.
The following items must be submitted prior to licensure:
1. Proof of the director's enrollment in the lead training course.
2. Photos of a 4 feet portable wall used as a barrier between the infant nap room and play room.
A 90-day provisional license shall be granted upon submission of the above items. The provisional license cannot be extended. Prior to granting a regular license, the applicant shall provide proof of Preventative certification to complete the requirement for current Preventative Health Practices. |
