13 | On September 13, 2023, at 1:40 PM, Licensing Program Analyst (LPA) Anastasia Flores, met with Director of Education, Kinglsey Bouldin to deliver the findings of the above stated allegations. On August 31, 2023, at 1:43 PM, LPA conducted a health and safety inspection, and no immediate concerns were observed. Copies of staff roster, photos of infant/toddler playground, and unusual incident report dated 9/05/23 were obtained. LPA reviewed staff files and conducted interview with Site supervisor.
On August 23, 2023, this agency received allegation that staff allow day care children to use equipment that is not age appropriate. Interview with staff #1 (S1) disclosed that the playground in question is currently not being used due to the equipment is not age appropriate. LPA observation on 8/31/23 revealed the playground was taped off and was not being used. Records received 09/05/23, revealed that the playground in question has not been used since July 1, 2023 and the playground was made inaccessible to children in care. (continued on next page) |
