This is an amendment to a report delivered on 02/02/2022.

Licensing Program Analysts (LPAs) Jeanette Sanchez and Ana Noble arrived at the facility to provide an amended report. LPAs toured the facility and took census.

LPAs met with Licensee Drucilla Craven to review the amended report and explain the dismissed citation. Hard copies were signed for the licensee and for the file.

An exit interview was conducted, and this report was reviewed with the licensee Drucilla Craven. Appeal rights were discussed and provided during the exit interview.

A notice of site visit was given and must remain posted for 30 days.
