On 05/19/2023 at 8:55 AM Licensing Program Analyst (LPA) Susan Brewer, arrived at the facility to conduct a 1 year required annual inspection. LPA was greeted and granted entry by Lead Teacher Victoria Valadez. LPA toured the facility, inside and out, reviewed records, and observed and/or discussed the following: Program Specialist Luz Romero, joined the inspection at 9:41 AM. NOTE: SUMMER PROGRAM OPERATION AT THIS SITE IS CONTINGENT UPON FISCAL FUNDING ALLOCATIONS; SUMMER HOURS ARE MON-THURS 8AM-11AM. THE DISTRICT HAS DETERMINED THAT THE 2023 SUMMER PROGRAM WILL BE PROVIDED AT AN ALTERNATE LOCATION.

Normal days and hours of operation: HEAD START OPERATES: TUE- FRI FROM 7:30AM TO 11AM & 11:45AM TO 3:15PM. STATE PRESCHOOL OPERATES MON-FRI FROM 8AM TO 11AM & 11:45AM; TO 2:45PM.

The inspection consisted of reviews of the following domains: Food Service; Reporting Requirements


Physical Plant; Care and Supervision; Children Records; Staff Records; Staffing Ratio and Capacity and
Personal Rights. The inspection found the facility to be in compliance in these domains.
The licensee is asked to update the following documents, if applicable, and submit to licensing within 30 days on or before 06/19/2023:

1.LIC 500 Personnel Report
2. LIC 610 Emergency & Disaster Plan
3. Parent Handbook/ Program Curriculum/Admission policies and procedures/ fee schedule (only if changes have been made)
4. LIC 309 Administrative Organization
5. LIC 308 Designation of Administrative Responsibility
6. Lead testing analysis report for outlets in classroom 2, post re-sampling and testing.
7. Waiver request for Preschool Children to share restrooms with elementary school children.

The following items have been posted and are updated where necessary:

· The facility is operating within the terms of the license.
· Ratios were met and appropriate supervision was provided during this inspection. LPA took a census of 18 children supervised by 4 staff.
· Classrooms are clean, free of hazards and equipped with age-appropriate furniture and equipment in good condition.
· No weapons stored at the facility on 05/19/2023
· There are no accessible bodies of water present 05/19/2023. All wading pools or similar product must be emptied immediately after use and stored in an upright position.
· Medications are stored where inaccessible to children in the School Sites Health Office, on site.
· Hazards are stored where inaccessible to children which include: Disinfectants, cleaners and other items that are dangerous. Poisons and toxins are locked on 05/19/2023.
· All floors are clean and safe on 05/19/2023.
· Bathrooms were observed to be safe, sanitary and in operating condition
· Outdoor activity areas/playgrounds are enclosed by appropriate fences, free of hazards and are supplied with age and size appropriate equipment in good condition.
· Food preparation area is clean, free of litter, rubbish and free of rodents and other vermin
· Measures are taken to keep the facility free of flies, other insects and rodents. The school maintenance provides monthly service.
· Food is stored appropriately and protected from contamination at the district office off site.
· Storage containers for solid waste, including moveable bins-have tight-fitting covers in good repair.
· Menus are posted at least one week in advance in a place visible by the child’s authorized representative, dated and kept on file for 30 days, and made available upon request. All food is stored and prepared off site and brought to the facility from the district office-kitchen.

·Uncontaminated drinking water shall be readily available both indoors and out and provided by bottled water to refill jugs for personal drinking cups and dixie cups. The facility was issued a citation on 05/16/2023 with a POC date of 06/16/2023, for detected lead exceedance in classroom 2. Program specialist informed the LPA that the facility classrooms 1 and 2, are going through a remodel as of 05/24/2023. Room 2 outlets A & B identified as having lead exceedances have been replaced and are pending retesting.
· The areas around or under high climbing equipment, swings, slides, and similar equipment shall be cushioned with material that absorbs a fall: Facility has wood chips, natural grass and sand.
· Sign in/Sign out record was reviewed and meets regulation requirements on 05/19/2023.
· A Staff member is present with current Pediatric CPR/First Aid which expires on 03/2025.
· Opening and closing staff member’s CPR/First Aid expires on 06/2023.
· Director is new and pending completion of Health, Safety, Nutrition and Lead Training. A qualified teacher has completed the Health, Safety and Nutrition course, No lead in Spring 2020.
· Staff qualifications and files were reviewed and are complete 05/19/2023.
· Staff have received on the job training for housekeeping, sanitation and universal health precautions
Licensee was informed the Department has inspection authority per Health and Safety Codes.

· Children files were reviewed and are complete on 05/19/2023.
· Documentation of fire & earthquake drills to be conducted every six months: Last drill on 05/03/2023 @ 8:50 AM for Classes 1 and 2. A fire clearance was issued on 08/25/2011. L. Romero, tested the carbon monoxide and smoke alarms during the inspection. The fire extinguishers were last inspected on 06/28/2022 and needles are in the green.
· AB2370 – Lead Exposure, day care facilities, effective January 1, 2019 –
The bill requires that all Child Care Centers, operating in a building constructed before January 1, 2010, shall have their drinking water tested for excessive lead levels, on or after January 1, 2020, but no later than January 1, 2023. Child Care Centers must thereafter test their drinking water every five years after the date of the initial test.
· AB605 – Child day care facilities, birth to school-age license; Effective January 1, 2021
· AB2960 – Child care and development services, online portal – Effective June 20, 2022 - This bill requires the State Superintendent of Public Instruction (SSPI), within the California Department of Education (CDE), to develop and post on CDE's website a comprehensive child care and development services online portal for families and providers by June 30, 2022.

This facility provides Incidental Medical Services – IMS. LPA reviewed children’s files and verified storage of medication and equipment/supplies, are stored in the Health Office at the School site. Medication is administered by a registered health assistant. The licensee Program Specialist Luz Romero, was reminded that all adults 18 and over, working employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.
