Licensing Program Analysts, Patricia Berry and Marlene Wong, conducted a Case Management visit to the facility to clarify the children involved on Complaint Control Number 09-CC-20190408135132, dated 04/08/19. Analysts met with Brandie Rubalcaba, Director.

A new Confidential Names (LIC 811), with the names of the children involved, was discussed and given to Ms. Rubalcaba.

An exit interview was conducted with Ms. Rubalcaba and a copy of this report was left a the facility.
