25 | A case management visit is being conducted in response to the receipt of an unusual incident report (UIR) from the facility. The UIR was received by the licensing agency on 06/10/2019. It indicates that on 06/07/2019, staff member S1 yelled at a daycare child C1.
LPA conducted interviews with pertinent parties, reviewed and obtained facility records and statements. The information gathered through interviews were that although staff S1 may have used a firm tone of voice, child C1 perceived it as yelling and stated they felt humiliated.
Based on the information gathered, the following violations have been identified: 101223(a)(1) Personal Rights. The licensee shall ensure that each child is accorded the following personal rights: To be accorded dignity in his/her personal relationships with staff and other persons.

A NOTICE OF SITE VISIT WAS ISSUED AND LPA VERIFIED THAT IT WAS POSTED IN A PROMINENT LOCATION AT THE FACILITY BEFORE LEAVING. THE FACILITY STAFF UNDERSTANDS THAT IT MUST REMAIN POSTED FOR THE NEXT 30 DAYS ALONG WITH A COPY OF ALL TYPE A DEFICIENCIES (LIC9099D) CITED DURING THIS INSPECTION.
