25 | Licensing Program Analysts (LPAs) Elyse Jones and Claudia Caywood arrived at the facility to conduct a Case Management-Incident inspection in response to the receipt of an Unusual Incident report (UIR). The UIR was received by the licensing agency on May 9, 2023 via phone call. The UIR documented an incident involving a child being left alone, without supervision, on the preschool playground. A facility tour was conducted and census were taken. Interviews were conducted and documentation was collected.
The following information was gathered/disclosed by interviews and documents obtained: Around 4:00PM there were two staff outside on the preschool playground with nine children. Staff 1 was by the door holding a crying child while attempting to gather all the children to get in line to go back inside of the classroom. Staff 2 observed Child 1 under the slide and called him/her to the line but did not realize he/she never got in line due to tending to another child who was also crying. As the class was going inside the Director was conducting census and when she went out to the School Age playground she noticed Child 1 outside under the slide. The Director immediately walked to the preschool playground and walked the child to the classroom. Staff 1 and Staff 2 stated, the incident occurred due to them being distracted with the crying children, not completing the Name to Face and rushing to get PM diapers done before the children went home. As a result, Staff 1 and Satff 2 were given additional training on transitioning children to/from the playground.
Based on the information gathered, facility failed to comply with Care and Supervision per regulation section 101229(a)(1) Care and Supervision, which specifies: No child(ren) shall be left without the supervision, including visual observation, of a teacher at any time except as specified in sections 101216.2(e)(1) and 101230(c)(1).
