25 | LPA, Joanne Domingo arrived at the facility on a case management visit to follow-up on an unusual incident report submitted by the facility on 7/24/19. The incident occurred on 7/19/19. LPA took census, and met with Assistant Director, Selena Estrada who arrived shortly from break to discuss the reported incident.
At the time of visit, LPA toured the facility outside play area along the back facing the business complex. Per Ms. Selena the playgrounds facing Trautwein were not used the week of 7/15/19 - 7/19/19 due to ants. During the visit, LPA also spoke with staff members. It is unknown what staff member or child was involved. There is only one staff member that fits the description of the reporting party's information, however that staff member does not recall any unusual incidents from earlier in the day on 7/19/19. The staff member had already gone home for the day at 5:15pm.
In addition, CSR records revealed the following classes were outside at the following times: 2's class from 4:20pm - 4:45pm; the 3's class from 3:35pm - 4:00pm; and the school age class from 4:16pm - 4:53pm. There were no children outside on the back playground after 4:53pm.
Based on the information obtained during the visit, there appeared to be no violations of Title 22 Regulations pertaining to the reported incident.
An exit interview was held with Assistant Director - Selena Estrada. A Notice of Site visit was issued, along with a copy of this report. This report shall be public record for three years. |
