On date and time listed, Licensing Program Analysts (LPAs) Taadhimeka Zeigler, Elyse Jones, and Nelson Zuniga arrived at the facility to initiate a complaint investigation into the above allegations. LPAs met with Director, Angela Green. LPAs discussed the purpose of the visit. The facility was toured and the census was taken.

The investigation included staff interviews, observation, and the review of pertinent documentation.

Regarding the allegation that facility staff failed to meet the needs of the day care child, based on interviews conducted, children are taken to the restroom every one to two hours. Some children require more frequent restroom breaks. In between staff asking children if they have to use the restroom, children that are potty trained may and will ask to use the restroom. If a child asks to go to the restroom, they are taken to the restroom by staff. There have been instances when a child would not want to go to the restroom, when asked, but would shortly after have an accident. These accidents are usually due to the children not wanting to stop playing. Due to conflicting statements, LPAs were not able to corroborate the allegation.

Regarding the allegation that staff failed to supervise day care children, based on interviews, staff supervise day care children. During outside play time, teachers may talk to one another, but attention is not taken off of the children. Staff do not have extended conversations with one another while supervising the children. Staff are not allowed to use cell phones while they are working and supervising children. Cell phones are kept in the Director's office. LPAx observed the cell phones in the Director's office. Due to conflicting statements, LPAs were not able to corroborate the allegation.

Regarding the allegation that drinking water is not accessible, based on interviews conducted and observation, drinking water is accessible. The water available for the two year old room is on the other side of the partition. Children will ask for water and staff will assist them in getting water. Due to conflicting statements, LPAs were not able to corroborate the allegation.

Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore, the allegations are UNSUBSTANTIATED.

No citations were issued today.

An exit interview was conducted with, Angela Green, and a copy of this report was provided.

A Notice of Site Visit was provided and LPAs verified its posting. It must remain posted for 30 days for public review.

A copy of this report must be made available to the public upon request for the next 3 years.
