Licensing Program Analyst (LPA's) Diana Brasel and Nelson Zuniga conducted an on-site investigation into the above allegation. LPA's met with Cindy Floyd to discuss the alleged allegation and gather information. Interviews were conducted on the above noted date with staff and children. The staff in quiestion denied the above allegation. Conflicting information was obtained from the interviews that were conducted on today's visit. The Department has investigated the above allegation and although it may have happened or been valid, there is not a preponderance of the evidence to prove that the alleged violaiton occurred. As of January 1, 2017, the term "inconclusive" is no longer used to refer to the outcome of certain complaint investigations. Such complaint allegations are now deemed "unsubstantiated". This document has not this change and for puposes of this complaint, investigation the Department's finding is that this allegtion was unsubstantiated.
Exit interview was conducted and a copy of this report was provided.
A Notice of Site Visit issued and shall be posted for 30 days.
Appeal rights were provided and a copy of this report must be available to the public for three years.
