On the date and time listed above, a complaint investigation was conducted by Licensing Program Analyst (LPA) Giselle Carbullido to amend the report dated 06/18/2019. During today’s visit, LPA toured the facility and census was taken, with 43 children present. LPA met with Melissa Almanza, Assistant Principal to deliver the findings.

Due to additional information received, this agency has reviewed the complaint of the facility staff not adhering to the admission agreement and facility staff failing to provide children’s authorized representative access to children’s records.

Facility Director states she provided parent a copy of children’s files on 06/10/19.

Facility email correspondence from Facility Executive Director confirms denial of access to children’s records under Department of Education codes, however per California Code of Regulations, Title 22, Div. 12. 101221(e) states, “A child's records shall also be open to inspection by the child's authorized representative.”

Based on additional information received, the allegation of the facility failing to provide children authorized representative access to children's records has been substantiated.

Appeal rights issued and discussed with licensee.

A copy of this report must be made available to the public upon request for three years.
