25 | On the date and time listed, Licensing Program Analyst (LPA) Nasha King and Licensing Program Manager (LPM) Carlos Martinez arrived at the facility to conduct a Case Management-Incident follow-up visit on an Unusual Incident Report (UIR) received by the Department on 02/28/2022. LPA and LPM met with Alejandra Bautista (Site Supervisor) to discuss the incident. A tour of the facility was granted, and census was conducted.
Per Ms. Bautista, she was not able to provide LPA with the details of the incident because she was not present when the incident occurred. However, Ms. Bautista informed LPA King that Mr. Rallion (Principal) would be able to provide additional information regarding the incident. LPA King spoke with Mr. Rallion via telephone who relayed that the investigation is still ongoing and that the information that was submitted on the Unusual Incident Report is the latest information. The staff member who was involved in the incident has been removed from the facility pending an investigation. The incident is being investigated by the LaQuinta Police Department.
LPA King has determined that the facility took the necessary steps to ensure the health and safety of the children in care by removing the staff member from the classroom/facility pending an investigation and also reporting the incident to the Department (Community Care Licensing).
Based on the information gathered, there appears to be no violations of Title 22 Regulations found at this time. An exit interview was conducted, and a copy of this report was provided to Alejandra Bautista (Site Supervisor).
There were no deficiencies cited during this inspection. A Notice of Site Visit was issued and the Licensee understands that it must remain posted for 30 days.
