25 | On Thursday, February 12, 2025 Licensing Program Analyst (LPA) Jesse Gardner conducted an unannounced Case Management inspection to follow-up on an Unusual Incident Report (UIR) submitted to Community Care Licensing CCL on December 18, 2024. LPA met with Principal Maria Moore and toured the facility. LPA had previously interviewed the director, four staff members, Child 1 (C1), a parent, and reviewed documentation during this investigation.
On December 18, 2024, CCL received information via UIR that on December 11, 2024, C1 told their parent that Staff 1 (S1) had put their face down in the restroom. Through LPA’s investigation, it was determined through record review, and interview, that 2 staff including S1, were in the classroom on the day of incident. 2 of 2 staff denied any incident that would be detrimental to C1 including putting their face down in the restroom, or otherwise, in the toilet. Interview with Parent (P1) stated that upon pick up, C1’s clothes were not the clothes they arrived at the day care in. P1 stated that the clothes were wet and needed to be washed upon reaching their home. Interview with S1 stated that C1’s clothes were not wet on the day of the alleged incident, and therefore, C1 was not needed to be changed. S1 relayed that C1 at times, needs assistance getting themselves correctly onto the toilet. According to S1, this was the case on the day of the alleged incident. S1 relayed that while assisting C1 in the restroom, S1 took hold of C1 under their armpits and went to lift C1 up onto the toilet to assist C1 with proper placement. While doing this, S1 briefly lost their balance, but caught themselves on the wall of the stall before they fell into C1. This action did; however, slightly make C1 not correctly position themselves on the toilet properly. S1 did state; however, that C1 told S1 that the incident hurt C1.
