(1) Licensing Program Analyst (LPA) Kim Leung arrived at the facility to conduct an annual inspection. LPA toured the facility, inside and out, records were reviewed and the following was observed and/or discussed:
Normal days and hours of operation are: Monday through Friday from 6am to 6pm. During inspection, licensee stated that she would submit application to change the days and hours of operation to open on Saturday as well. Licensee stated that on Saturday, she would operates the facility as a small family day capacity with no more than 8 children in care as she would not have any assistant on Saturday.
OFF-LIMIT AREAS INCLUDE: All 4 bedrooms, laundry room and garage
· The facility is operating within the licensed capacity and appropriate ratios
· The Licensee and 3 assistants present in the home and have ensured that children in care are supervised at this time
· A working telephone is present
· Fully-charged fire extinguisher is present. Smoke detector and carbon monoxide detector are present and were tested by the assistant during this inspection and found to be functioning at the time.
· All hazardous items are inaccessible, this includes: detergents, cleaning compounds, medications and other items which could pose a danger to children
· There is a properly barricaded fire place
· No guns or weapons present as stated by the Licensee. Licensee understands all guns, weapons and ammunition must be key-locked separately and made inaccessible per Title 22 regulations.
· Stairs leading to the front living room are barricaded at this time
· Home is clean and orderly, with air-conditioning and ventilation for safety and comfort
· Safe and appropriate toys and equipment are present for both indoor and outdoor activities.
· Outdoor play areas are fenced
· Verification of control of property on file (owner)
· License, Facility Sketch, Emergency Disaster Plan & Notification of Parent’s Rights poster are posted
· Pediatric CPR and First Aid Card expire in September 2020 (licensee), on 8/11/2020 (assistant Brandy Pacheco) and 12/31/2020 (assistant Gretel Castillo)
· Health & Safety Certificate - completed on 10/13/2007

- Responsibilities of being a mandated reporter
- Baby walkers, bouncy seats, exersaucers and other similar items are prohibited
- The licensee is urged to visit the U.S. Consumer Product Safety Commission webpage at www.cpsc.gov to ensure that equipment purchased for the day care has not been recalled.
- Once licensed, the Notice of Site Visit must be posted at the entrance of the facility for a period of 30 days. If a serious violation is cited, a copy of the licensing report (LIC809/LIC9099) must also be posted for 30 days. A civil penalty of $100 per violation will be assessed for noncompliance.

v - AB 1207 – Mandated Child Abuse Reporting: Child Day Care Personnel Training, beginning January 1, 2018 – In accordance with California Health and Safety Code Section 1596.8662 – requires all licensed providers, applicants, directors and employees to complete training as specified on their mandated reporter duties and to renew their training every two years. Applicants must meet requirements as a precondition to licensure. Existing licensees must meet requirements by March 30, 2018. New employees shall have 90 days to complete training as required. This training requirement may be directly met by using the Department’s Office of Child Abuse Prevention (OCAP) online training modules. The OCAP modules are free of cost and available at: http://www.mandatedreporterca.com/ and are provided in English and Spanish. If no training is made available in a required person’s primary language then those persons shall be exempt from this requirement

See LIC809D for cited deficiencies. Appeal rights discussed and a copy of this report was provided to the licensee on this date. During the exit interview, licensee Nancy Estrada confirmed that there are no Registered Sex Offenders living in the facility and/or using the facility address for their mailing address. A NOTICE OF SITE VISIT WAS ISSUED AND IS TO BE POSTED IN A PROMINENT LOCATION AT THE FACILITY FOR THE NEXT 30 DAYS. A copy of this report was provided to the licensee on this date and must be made available to the public upon request for the next 3 years.

· There is an in-ground pool in the backyard. The pool is properly fenced with 5-foot tall mesh fencing with a self-closing and self-latching gate that opens away from the pool. Licensee states that she understands that the mesh fencing would be properly in placed at all times during child care hours. Licensee understands all bodies of water including ponds, above ground pools & spas, in-ground pools & spas, and some fountains must be properly covered or fenced per Title 22 regulations. The Department must be notified before and after installation of the above types of bodies of water. In addition, all wading pools or similar product must be emptied immediately after use and stored in an upright position.
· Documentation of fire drills on file. The most recent disaster drill was conducted on 6/22/2019 per drill log.
· Each child’s file contains a copy of the emergency information card with required information
· Criminal record clearances are required prior to all adults living or working in a Family Child Care Home. A civil penalty of $100.00 per day the person has been present, may be assessed. All individuals subject to a criminal record review shall obtain a criminal record clearance or exemption prior to working, residing or volunteering in a licensed home. This was verified on 9/12/2019.
· The Department was granted inspection authority as required by the Health and Safety Code
· Facility is not currently providing IMS at this time. Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual – Regulation Interpretations and Procedures for Family Child Care Homes Section 102417. Prior to any IMS is provided, a Plan for Providing IMS must be submitted to the Department.
