1 2 3 4 5 6 7 8 9 10 11 12 13 14 15 16 17 18 19 20 21 22 23 24 25	On the above date and time Licensing Program Analyst (LPAs), Susan Brewer and James Wilkerson, arrived at the facility unannounced for the purpose of conducting an annual inspection as part of a compliance review. The directors were not present at the upon arrival. A teaching Staff 1 granted LPAs entry into the facility. The LPAs conducted a COVID-19 pre-screening. This is a not combination childcare center. A tour of the inside and outside of the facility was granted and the following was observed and/or noted: Director Laura Wood arrived at approximately 8:58 AM, to finish the annual inspection. DAY AND HOURS OF OPERATION: MONDAY - FRIDAY, 7:30 AM TO 6:00 PM · The following items were posted and updated where necessary: - License - Emergency Disaster Plan (LIC610) and Earthquake Preparedness Checklist (LIC9148) - Parent’s Rights Poster (PUB393) - Personal Rights (LIC613A) - Child Car Seat Law - Menu, No menu posted, children bring their own snacks and meals to school. · The facility is operating within the limits as stated on the license. · Ratios are being met during this inspection · Classrooms are adequately equipped with age and size appropriate furniture and equipment and free of hazards · There are no weapons present at the facility as stated by Laura Wood. · There are no accessible bodies of water present. All wading pools or similar product must be emptied immediately after use and stored in an upright position. · Drinking water is provided in the indoor activity space (add detail for each program if necessary) by Brita Filter to filter tap water supply filtered cups - and in the outdoor activity space by Brita Filter to filer tap water and disposable cups.

· Medications are stored where inaccessible to children
· Hazardous items are stored where inaccessible to children which include: Disinfectants, cleaning solutions and other items that are dangerous
· Poisons and toxins are locked and inaccessible to children
· All floors were observed to be clean and safe
· Bathrooms were observed to be safe, sanitary and in operating condition
· Playgrounds are NOT enclosed by appropriate fences and free of hazards. At approximately 8:56 AM during the inspection of outdoor area for the Pre-school East Playground, LPAs observed large gaps along the bottom perimeter of the wrought iron fencing that are more than 5" deep, between the foundation and fencing.
· Outdoor activity areas are supplied with age and size appropriate equipment in good condition, however a large shade tarp was observed by the LPAs to have large tears. The director stated they do have a new tarp that will replace the torn tarp.
· Food preparation area is clean, free of litter and rubbish and free of rodents and other vermin. Per director, no food prep is conducted, all lunches and snacks are brought in from home.
· Food is stored appropriately and protected from contamination
· All storage containers for solid waste were observed to have tight-fitting covers that are kept on, and in good repair.
· Sign in/Sign out record was reviewed and meets regulation requirements
· Disaster drills are conducted at least every six months – last drill conducted on 09/15/2021 at approximately 10:00 AM, with 47 children and 7 staff present, lasted 1:45 minutes.

A review of staff and children's records were conducted as part of this evaluation.
· Children’s records were found to be incomplete during this inspection, C2 missing LIC613A personal rights.
· Staff records review indicates that all staff present do NOT meet minimum qualifications for the position for which they were hired, Immunization records missing for Staff 3 and 5 during file review.
· A staff member is present with current Pediatric CPR/First Aid which expires on 08/15/2022.
· Opening and closing staff member’s CPR/First Aid expires on 08/15/2022.
· Director completed Health and Safety Training
· A review of staff records on this 12/20/2021 indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. The following items were discussed with the Licensee/Director during inspection:

· Menus shall be posted at least one week in advance in a place visible by the child’s authorized representative, dated and kept on file for 30 days, and made available upon request..
· The areas around or under high climbing equipment, swings, slides, and similar equipment shall be cushioned with material that absorbs a fall
· The Licensee was informed of their reporting requirements and provided with the Regional Office’s Unusual Incident Reporting email mailbox: UnusualIncidentReportsDO10@dss.ca.gov
· The Licensee can submit transfer forms to associate new individuals or to disassociate someone from your facility at: Associations_Disassociations858@dss.ca.gov

LPA discussed the safe sleep regulations with licensee [or facility representative] and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee [facility representative] of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Licensee Laura Wood, was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.
