On 6/7/2023 at 03:53 PM, Licensing Program Analysts (LPA) Claudia Caywood arrived at the facility to conduct a Case Management and place the licensee on inactive status. Upon arrival, LPA was met by Licensee, Brooke Thompson. LPA stated to the licensee the purpose of the visit. Licensee stated to LPA there were no children enrolled at the time of inspection.

Licensee agrees to be placed on Inactive status starting 6/7/2023 while they decide if they want to reopen up again sometime in August of 2023 . LPA handed them an inactive form to complete which was signed and dated by licensee.

No deficiencies cited.

An exit interview was conducted, and report was reviewed with the licensee, Brooke Thompson. A Notice of Site Visit was issued and is to be posted in a prominent location at the facility for the next 30 days.
