Licensing Program Analysts (LPAs) Laura Mejorado and Destinee Hogue visited the facility for another purpose on this date 10/29/2021. Upon arrival, LPAs met with Director Izabeela Morales and stated the purposed of the inspection.

Upon arrival LPAs observed one staff supervising 22 children. LPAs observed Preschool and School-age children commingling in the outside play area. This is a violation of the shared outside activity space waiver approved on 12/18/2013. Waiver states "preschool children to share the outside play ground area with the licensed school age program on a rotational schedule basis". The waiver was granted under the following conditions: 1. The outside activity schedule shall be maintained and posted in each classroom to ensure there is no commingling of the school age children with the preschool children. 2. The appropriate teacher child ratio shall be maintained at all times. This was not observed to be in compliance during todays inspection.

An exit interview was conducted with the Director, Appeal Rights were discussed and issued, a copy of this report was provided, and a Notice of Site visit was issued.

Upon receipt of this report, the Licensee shall post the Notice of Site Visit and any Licensing report documenting a type “A” deficiency. The report and the Notice of Site Visit shall be posted for 30 consecutive days. Failure to maintain posting as required, will result in an immediate $100 civil penalty. A copy of this report shall be provided to the parent/guardian of children currently enrolled by the next business day or immediately upon return. A copy of this report shall also be provided to the parent/guardian of any newly enrolled children for the next 12 months (1 year). The Acknowledgement of Receipt (LIC 9224 form must be maintained in each child’s file immediately upon receipt from parent. Licensee was provided with a copy of the Acknowledgement of Receipt of Licensing Reports (LIC 9224)
