25 | Licensing Program Analyst (LPA) Ana Noble arrived at the facility on a case management inspection to follow-up on an Unusual Incident Report (UIR) which occurred on August 13, 2019. The incident reported was that a portion of the playground canopy/shade structure had burned and playground is currently not being used at this time. LPA met with Nicole Shropshire, Lead Teacher, due to Nicole Fuentes, Director not being present during inspection. LPA toured the facility, took census, inspected the playground and playground structure. LPA took pictures of the playground structure. LPA also interviewed Ms. Shropshire and obtained documentation.
The information obtained and observations of LPA Noble indicate the incident took placed on August 13, 2019. The inspection of the shade structure and playground reveals that there is still shade being provided, due to the canopy/shade structure not completely burning (pictures taken of canopy and play structure). However, the residue of the material of canopy/shade structure melted onto the play structure and is not removable. The facility has another playground that is being used for the children on a schedule basis. The playground impacted will not be used until play structure is replaced. The Licensee is currently working on having the playground and canopy/shade structure replaced. The facility has provided documentation of the work that will be completed.
Based on all the information gathered, there is no violation of Title 22 Regulations. A Exit interview was conducted with facility, Ms. Shropshire, Lead Teacher, a Notice of Site Visit was issued and must be posted up at a prominent location at the facility for 30 days. A copy of this report was provided to the facility. This report must be made available at the facility for 3 years for public review upon request. |
