On February 10, 2022 Licensing Program Analysts (LPAs) Elyse Jones and Justin Giese conducted a Case Management-Other. Licensee was not home upon arrival. LPAs called the Licensee and she arrived five minutes later.

No children were present during the inspection. Current days and hours of operation are Monday-Friday 7:45AM-12:30PM.


Based on the above, the facility was found to be in compliance with Title 22, at this time.

A NOTICE OF SITE VISIT WAS ISSUED AND IS TO BE POSTED IN A PROMINENT LOCATION AT THE FACILITY FOR THE NEXT 30 DAYS.
