On 01/12/2024, Licensing Program Analyst (LPA) Lorena Valenzuela met with Children’s Lighthouse Learning Center Director Linda Scott to deliver the findings of the complaint investigation for the above allegations. The investigation included an inspection of facility and a review of facility records on 11/17/2023. In addition, LPA interviewed Director, and five staff.
On November 11, 2023, Community Care Licensing (CCL) received information that play equipment, specifically a trolley located in the playground of the facility, is in disrepair. LPA Valenzuela observed the trolley to be made of wood that showed areas of wear and large stains in the wood.
Based on interviews and observations, the preponderance of evidence standard has been met, and the allegation play equipment is in disrepair is substantiated. The facility is being cited under Title 22, Section 101239 (n)Fixtures, Furniture, Equipment, and Supplies. See deficiency report for citation cited.
An exit interview was conducted, and a copy of this report, LIC 9099-D, and appeal rights was provided to Director Linda Scott.
The Notice of Site Visit was provided, the licensee was reminded this notice must be posted for 30 days.

Director Linda Scott states will bring the trolley to good repair or will remove the equipment from the playground. Will send proof of corrections to the Department by due date 02/09/2024.

Based on interviews, observations, the facility did not ensure the play equipment (trolley) in the school age playground was in good condition. This poses a potential health, safety or personal rights risk to the children in care.

On November 11, 2023, Community Care Licensing (CCL) received information that facility is not being maintained in a sanitary manner and facility is not free from insects. Interviews conducted revealed that the facility uses a cleaning service that will clean all classrooms every day after the center is closed. In addition, staff clean counters daily. Confidential interviews revealed that there have been times when the cleaning services do not clean floors thoroughly. Interviews revealed, the Director has addressed the areas of concern with the cleaning services.
Regarding the allegation that the facility is free from insects, interviews revealed that the facility has had issues with mosquitoes in the outdoor area. It was reported children were being bitten by mosquitoes daily. Interviews revealed, the facility has contracted pest control services to address the issue and has provided documentation to the Department.
