On 02/01/2023 at 01:30PM Licensing Program Analysts (LPA) Giselle Carbullido conducted an unannounced visit regarding a complaint received concerning the above allegations. LPA met with Marie Lacoste to further discuss the complaint allegation(s) and deliver findings.
Lack of supervision resulting in children wandering away from the facility. It was reported two young children left the facility grounds by exiting through an opening in the fence, then walked alone along the street.
LPA interviewed 2 children and 2 staff. Children interviews reported a child left through an opening in the fence. Staff interviews reported a child did leave the facility grounds and acknowledged a passerby called the facility to inform them of children leaving through a hole in the fence.
On 01/30/23, the facility submitted an Unusual Incident Report (UIR) to the Department. The UIR states a child admitted to leaving the grounds by going through an opening in the fence.
Facility fence is in disrepair. It was reported the facility had a hole in the fence.
LPA interviewed 2 children and 2 staff. All interviews stated there was an opening in the fence. Staff interviews also reported the fence required repairs and that all programs were notified not to play in the front play yard until

repairs were completed. Repairs were completed over the weekend.

Record review revealed facility communication with Diego Landscaping for fence repair. The landscaping company replaced two fence boards.



This agency has investigated the complaint regarding the allegation. Based on interviews conducted the preponderance of evidence standard has been met, therefore the above allegations are found to be SUBSTANTIATED per California Code of Regulations 101416.5(b), Title 22, Division 12.

LPA Carbullido informed licensee facility representative, Marie Lacoste that this report dated 02/01/23 document(s) (2) Type A citation(s) which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care.

Also, LPA Carbullido informed the facility representative, Marie Lacoste to provide a copy of this licensing report dated 02/01/23 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

An exit interview was conducted, and appeal rights discussed. LPA Carbullido provided facility representative with a copy of this report, notice of site visit and appeal rights.

A copy of this report must be made available to the public upon request for three years.

Based on LPAs record review and interviews conducted the facility did not meet the section above in that a child was unsupervised after they left facility grounds through a hole in fence. This poses an immediate health and safety risk to children in care. An immediate $500.00 civil penalty has been assessed.
