32 | • An infant’s head must not be covered while sleeping.
• If an infant falls asleep before being placed in a crib, for example, in a provider’s arms or stroller, the provider must move the infant to a crib (or play yard for FCCHs) as soon as possible.
• Car seats will only be used for transportation and must not be used for sleeping within a child care facility.
• All pacifiers cannot have anything attached, such as a stuffed animal or a clip meant to attach the pacifier to the infant’s clothing.
• Providers must check on sleeping infants every 15 minutes and document their condition to check for signs of distress, which includes, but is not limited to labored breathing, flushed skin color, increase in body temperature, and restlessness.
• Each infant, up to 12 months of age, must have an Individual Infant Sleeping Plan (LIC 9227) on file, which will document the infant’s sleeping habits, usual sleep environment, and the infant’s rolling abilities.
- Baby walkers, bouncy seats, exersaucers and other similar items are prohibited
- The applicant is urged to visit the U.S. Consumer Product Safety Commission webpage at www.cpsc.gov to ensure that equipment purchased for the day care has not been recalled
- The Notice of Site Visit must be posted at the entrance of the facility for a period of 30 days. If a serious violation is cited, a copy of the licensing report (LIC809/LIC9099) must also be posted for 30 days. A civil penalty of $100 per violation will be assessed for noncompliance.

During the exit interview, the LICENSEE, Lourdes Felix, confirmed that there are no Registered Sex Offenders living in the facility and/or using the facility address for their mailing address.
The application for a Large Family Child Care Home will be submitted for approval with a maximum capacity of 12, or 14 with parent notification. Ratios discussed during the inspection.
An exit interview was conducted via FaceTime, LPA Robinson provided the Licensee with a copy of this report and Notice of Site Visit via email, LPA asked the Licensee to acknowledge receipt of the email. An electronic “read receipt” was also attached. The electronic read receipt of the emailed report acknowledges receipt of this report. A copy of this report was emailed to Licensee during this Tele-inspection on May 28, 2021. |
