May 27, 2021 Due to COVID-19 Licensing Program Analyst (LPA) Sharleen Robinson conducted a Tele inspection with Licensee Lourdes Felix via FaceTime, licensee applied to increase her capacity to that of a Large Family Child Care Home. COVID-19 precautionary measures discussed and technical assistance provided during the inspection. The Licensee toured LPA through the facility, inside and out, no children in care, records were reviewed and the following was observed and discussed:

Normal days and hours of operation are: 24 hours Monday-Saturday (Licensee understands she cannot provide 24 hours consecutive care to children)
Off limit areas are: All bedrooms and the detached garage in the backyard.

· Fire Clearance granted 5/28/21
· The licensee is present in the home and ensures that children in care are supervised at
all times
· Fire extinguishers, smoke detectors, and carbon monoxide detectors meet State Fire
Marshall standards and were tested by the licensee during this inspection.
· Fireplaces and open face heaters are screened to prevent access by children
· Storage areas for poisons, detergents, cleaning compounds, medications and other items which could pose a danger to children are stored where they are inaccessible to children
· All poisons are locked
· No gun or weapons per Licensee. LICENSEE UNDERSTANDS ALL GUNS, WEAPONS AND AMMUNITION MUST BE KEY-LOCKED SEPARATELY AND MADE INACCESSIBLE PER TITLE 22 REGULATIONS.
· The home is kept clean and orderly, with heating and ventilation for safety and comfort
· Each child has safe, healthful, and comfortable accommodations, furnishings, and equipment
· No bodies of water observed. LICENSEE UNDERSTANDS ALL BODIES OF WATER INCLUDING

PONDS, ABOVE GROUND POOLS & SPAS, IN-GROUND POOLS & SPAS, AND SOME FOUNTAINS MUST BE PROPERLY COVERED OR FENCED PER TITLE 22 REGULATIONS. The Department must be notified before and after installation of the above types of bodies of water. In addition, all wading pools or similar product must be emptied immediately after use and stored in an upright position.
· The home has a current roster of children (Licensee sent LPA a copy)
· The home conducts fire and disaster drills at least once every six months, and documents the date and time of each drill 04/05/21 (Licensee sent LPA a copy)
· Licensee documents immunizations and maintains and updates records for children in care (Licensee sent LPA a copy of a child's file to review)
· Licensee provides the child's parent or representative with a copy of the Family Child Care Home Notification of Parents' Rights
· The licensee and other personnel, as specified, have completed training on preventive health practices which was completed on 9/8/2014 and updated on 3/27/21. CPR and First Aid expires 1/23.
· Criminal record clearances are required prior to all adults living or working in a Family Child Care Home. A civil penalty of $100.00 per day the person has been present, may be assessed. Resident and/or staff records reviewed on 5/28/21 indicate that all adults who require caregiver background checks have received all required clearances or exemptions. The Licensee can submit transfer forms to associate new individuals or to disassociate someone from your facility at:

· The licensee understands that upon notice of the Department to remove an individual from the home, pursuant to H&S Code 1596.871(c)(2), or to exclude an individual from the home, pursuant to H&S Code 1596.8897, the licensee immediately removes the individual and prevents them from returning to the home or having contact with children in care
· Children are not left in parked vehicles and the licensee understands that this is prohibited
· The licensee understands that any authorized employee of the Department may enter and inspect any place providing personal care and services at any time, with or without advance notice.
· The Licensee was informed of their reporting requirements and is provided with the Regional Office’s Unusual Incident Reporting email mailbox: UnusualIncidentReportsDO09@dss.ca.gov
· Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual – Regulyation Interpretations and Procedures for Family Child Care Homes Section 102417. - AB2231 (2016) – Increased Civil Penalties, effective July 1, 2017 –
- AB 1207 – Mandated Child Abuse Reporting: Child Day Care Personnel Training, beginning January 1, 2018 Licensee's certificate is on file.
- AB2370 – Effective January 1, 2019 – Lead Poisoning training requirements Licensee obtained Lead training certificate on file.
- Effective January 1, 2017 – Children under 2 years of age shall ride in a rear-facing car seat unless the child weighs 40 or more pounds OR is 40 or more inches tall. For additional information regarding car seat laws see www.chp.ca.gov
- Safe sleep was discussed:
The following is a summary of the new safe sleep requirements:
• Infants up to 12 months of age must be placed on their backs to sleep, unless there is a medical exemption from a licensed physician that allows for an alternative sleep position.
• Cribs must be free from all loose articles and objects, including blankets and pillows.
• Mattresses must be firm and include a tight-fitted sheet.
• Infants must not be forced to sleep, stay awake, or stay in the sleeping area. • Infants must not be swaddled while in care.

• An infant’s head must not be covered while sleeping.
• If an infant falls asleep before being placed in a crib, for example, in a provider’s arms or stroller, the provider must move the infant to a crib (or play yard for FCCHs) as soon as possible.
• Car seats will only be used for transportation and must not be used for sleeping within a child care facility.
• All pacifiers cannot have anything attached, such as a stuffed animal or a clip meant to attach the pacifier to the infant’s clothing.
• Providers must check on sleeping infants every 15 minutes and document their condition to check for signs of distress, which includes, but is not limited to labored breathing, flushed skin color, increase in body temperature, and restlessness.
• Each infant, up to 12 months of age, must have an Individual Infant Sleeping Plan (LIC 9227) on file, which will document the infant’s sleeping habits, usual sleep environment, and the infant’s rolling abilities.
- Baby walkers, bouncy seats, exersaucers and other similar items are prohibited
- The applicant is urged to visit the U.S. Consumer Product Safety Commission webpage at www.cpsc.gov to ensure that equipment purchased for the day care has not been recalled
- The Notice of Site Visit must be posted at the entrance of the facility for a period of 30 days. If a serious violation is cited, a copy of the licensing report (LIC809/LIC9099) must also be posted for 30 days. A civil penalty of $100 per violation will be assessed for noncompliance.

- The Duty Officer is available to answer questions Monday – Friday at 1-844-LET-US-NO (1-844-538-8766).

During the exit interview, the LICENSEE, Lourdes Felix, confirmed that there are no Registered Sex Offenders living in the facility and/or using the facility address for their mailing address.

The application for a Large Family Child Care Home will be submitted for approval with a maximum capacity of 12, or 14 with parent notification. Ratios discussed during the inspection.

An exit interview was conducted via FaceTime, LPA Robinson provided the Licensee with a copy of this report and Notice of Site Visit via email, LPA asked the Licensee to acknowledge receipt of the email. An electronic “read receipt” was also attached. The electronic read receipt of the emailed report acknowledges receipt of this report. A copy of this report was emailed to Licensee during this Tele-inspection on May 28, 2021.
