25 | On 04/15/21 at 02:00pm Licensing Program Analyst (LPA) Justin Giese conducted a Tele-inspection to the facility to follow up on an Unusual Incident Report (UIR) that was submitted to the Regional Office on 04/07/21. Due to the executive order issued by Governor Newsom on March 16, 2020 regarding COVID-19 pandemic, this investigation was conducted via Tele-inspection. LPA met with Facility Director Lyndy Moore to discuss the purpose of the visit. LPA took census, reviewed records, toured the facility (virtually) and conducted interviews with staff.
On April 07, 2021 Facility Director self-reported an incident that occurred with Child #1 on April 6, 2021. UIR stated Child #1 was crying and distraught while in his classroom and playground. Child #1’s behavior was observed by three staff members. Child #1 disclosed to Staff and Director “they were picturing their teacher’s private parts and they were thinking about seeing them naked and touching their private parts together.” UIR stated Child #1’s conversations with staff were discussed with their Parent/Guardian when child was picked up from the facility. UIR stated Child #1’s Parent/Guardian emailed Facility Director later that same evening stating Child #1 was crying and distraught due to children inappropriately touching one another at the facility. Parent/Guardian also indicated Child #1 would be dis-enrolled.
LPA conducted interviews with Facility Director and Staff during the tele-inspection. Director and Staff interviews both corroborate information provided in the UIR. Staff and Director stated that children do not inappropriately touch one another. No allegations or occurrences of inappropriate touching have been recorded at the facility.
