March 19, 2021 Due to COVID-19, Licensing Program Analyst (LPA) Sharleen Robinson conducted a Licensee initiated Case Management Tele-inspection with Luz Romero, Early Childhood Services Specialist. LPA met with Luz Via FaceTime. The facility has been closed since March of 2020, during the closure the facility was remolded. In the infant room a wall was removed to extend the activity area, storage closet added, and isolation room created. In the toddler room windows were replaced, an additional kitchen sink was added. Rubber material added to the playground and a shade structure was added.

LPA inspected the facility inside and out no hazards observed during the inspection. COVID-19 precautionary measures discussed and technical assistance provided during the inspection. Safe sleep was discussed:

Safe Sleep:
The following is a summary of the new safe sleep requirements:
• Infants up to 12 months of age must be placed on their backs to sleep, unless there is a medical exemption from a licensed physician that allows for an alternative sleep position.
• Cribs must be free from all loose articles and objects, including blankets and pillows.
• Mattresses must be firm and include a tight-fitted sheet.
• Infants must not be forced to sleep, stay awake, or stay in the sleeping area. • Infants must not be swaddled while in care.
• An infant’s head must not be covered while sleeping.

• If an infant falls asleep before being placed in a crib, for example, in a provider’s arms or stroller, the provider must move the infant to a crib (or play yard for FCCHs) as soon as possible.
• Car seats will only be used for transportation and must not be used for sleeping within a child care facility.
• All pacifiers cannot have anything attached, such as a stuffed animal or a clip meant to attach the pacifier to the infant’s clothing.
• Providers must check on sleeping infants every 15 minutes and document their condition to check for signs of distress, which includes, but is not limited to labored breathing, flushed skin color, increase in body temperature, and restlessness.
• Each infant, up to 12 months of age, must have an Individual Infant Sleeping Plan (LIC 9227) on file, which will document the infant’s sleeping habits, usual sleep environment, and the infant’s rolling abilities.
CHILD CARE CENTER SPECIFIC REQUIREMENTS
• Half-walls or transparent walls may be used when constant audial and visual supervision can be maintained.
• In the event a separate room is used as a designated infant sleeping area, without transparent walls or half-walls, staff must adhere to the staff-infant ratio requirements, as specified in Section 101416.5(d).
Each infant, up to 12 months of age, must have an Individual Infant Sleeping
Plan (LIC 9227) on file, which will document the infant’s sleeping habits

The Early Childhood Services Specialist has requested to use the infant room for enrolled toddlers while the toddler room is undergoing mold remediation. Currently the toddler rooms are not being use. Early Childhood Services Specialist has been advised to request a waiver in writing. The room cannot be used until there is an approved waiver.

An exit interview was conducted via FaceTime, LPA Robinson provided the Early Childhood Services Specialist with a copy of this report and notice of site visit via email, LPA Asked the Early Childhood Services Specialist to acknowledge receipt of the email. An electronic “read receipt” was also attached. The electronic read receipt of the emailed report acknowledges receipt of this report. A copy of this report was emailed to the Early Childhood Services Specialist during this Tele-inspection on March 19, 2021.
