32 | It was also alleged that the facility had an authorized representative to sign child in and out when child did not attend. Information obtained during the course of the investigation revealed some parents signed by mistake for the week ending on 07/05/19. Other children’s signing sheets were updated by staff per parents request to reflect vacation leave or sick leave. Per site supervisor, parents were explained at pick up to correct signing sheets due to the confusion with the holiday of 07/04/19.
The information obtained during the course of the investigation is conflicting, therefore, based on the information provided during this investigation, it cannot be determined whether the facility staff violated admission procedures regarding the above allegations.
Although the allegation(s) may have happened or are valid, there is no preponderance of evidence to prove the alleged violation(s) did or did not occur, therefore the allegation(s) is deemed Unsubstantiated at this time.
No deficiencies cited at this time. An exit interview was conducted, and a copy of this report was provided to Lead Teacher, Alejandra Inzunza, on this date. Notice of Site Visit was posted and must stay posted for 30 days. |
