Licensing Program Analysts (LPAs) Blanca Ruiz-Silva, John Huynh and License Program Manager (LPM ), Aaron Ross arrived at the facility to conclude an investigation into the above allegation, prior visit was made on 07/22/19. LPAs and LPM met with lead teacher, Alejandra Inzunza and stated the purpose of the investigation. Site Supervision, Laura Diaz Belman was not present at the facility during the time of the inspection. The center was toured and a census was taken.

It was alleged that on or about the first week of 07/2019, Facility staff failed to follow admissions agreement and facility staff had authorized representative to sign a child in and out when child had not attended. During the initial inspection on 07/22/19, LPA Blanca Ruiz-Silva conducted interviews with Staff present at the facility, records were reviewed and additional interviews were conducted a sample of parents/authorize representatives. It was disclosed by Site Director, Laura Diaz Belman that the facility closed on Friday 07/05/19. Site Director explained that facility was closed after confirming with all the parents that they will not be attending due to the parents going on vacation or not coming to the center.

It was also alleged that the facility had an authorized representative to sign child in and out when child did not attend. Information obtained during the course of the investigation revealed some parents signed by mistake for the week ending on 07/05/19. Other children’s signing sheets were updated by staff per parents request to reflect vacation leave or sick leave. Per site supervisor, parents were explained at pick up to correct signing sheets due to the confusion with the holiday of 07/04/19.

The information obtained during the course of the investigation is conflicting, therefore, based on the information provided during this investigation, it cannot be determined whether the facility staff violated admission procedures regarding the above allegations.

Although the allegation(s) may have happened or are valid, there is no preponderance of evidence to prove the alleged violation(s) did or did not occur, therefore the allegation(s) is deemed Unsubstantiated at this time.



No deficiencies cited at this time. An exit interview was conducted, and a copy of this report was provided to Lead Teacher, Alejandra Inzunza, on this date. Notice of Site Visit was posted and must stay posted for 30 days.
