Licensing Program Analysts (LPAs) James Wilkerson and Joanne Domingo arrived at this facility to conduct an investigation into the above allegations. LPAs toured the facility and conducted census. LPAs conducted interviews with staff as well. There were allegations that the children's refrigerator was not clean and that staff do no sanitize between transitionings between classrooms. During the tour of classroom #6, LPAs observed a dark bown spilled substance on the bottom of the inside of the refrigerator that has been there for longer than one day as evidenced by how it was dried and caked. LPAs also observed classroom #3 transitioning to another classroom with the teacher and nine children and the toys that were being played with by children had not been sanitized prior to leaving the classroom. Based on the observations by LPAs during the tour of the facility, the preponderance of evidence had been met and the above allegations are SUBSTANTIATED. See LIC 9099D for defiiencies cited.

An exit interview was conducted, a Notice of Site Visit was conducted and a opy of this report was provided to the facility on this date.

Director, Doreen Blake agrees to have the refrigerator in classroom #6 cleaned and submit a photo and have a staff meeting on proper sanitizing with a signed agenda to Community Care Licensing by 03/14/21.

children's refrigerator in classroom #6. Children rotate into diffferent classrooms every 40 minutes for different themes. LPAs observed the items in classroom #3 not being sanitized prior to transitioning children into a different classroom. This is a potential risk to the health and safety of children in care.

Licensing Program Analysts (LPAs) James Wilkerson and Joanne Domingo arrived at this facility to conduct an investigation into the above allegation. LPAs toured the faciity and conducted census. There was an allegation that staff are using their bare hands when serving snacks/lunch to children and this is unsantiary. Staff who were interviewed deny this and stated that today's snack (Chex Mix) is being scooped out of large container with a cup and placed on paper towels in front of the children. A child who was interviewed stated that her teacher served him/her Chex Mix with his/her bare hands. Whille staff was not wearing gloves it cannot be proven at this time if they had washed their hands and if they are indeed using cups. Per the menu for the facility snack provided also include apple slices, Nilla Wafers, Goldfish, Cheerios and Saltineson different dates. LPAs cannot prove that staff are washing hands prior to serving in a santiary manner or not..
Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the allegation did or did not occur, therefore the allegation is UNSUBSTANTIATED.
An exit interview was conducted, a Notice of Site Visit posted and a copy of this report was provide to the facility on this dat.
