On 05/25/2022 at 9:05 AM Licensing Program Analyst (LPA) Susan Brewer arrived at the facility to conduct a 1 year required annual inspection. LPA was granted entry by Director Jane Osuna. LPA conducted a COVID-19 pre-screening, toured the facility, inside and out, reviewed records, and observed and/or discussed the following: LPA observed 5 classrooms operating and took a census of 54 children present and under supervision of 7 staff and 1 volunteer present. The facility follows a fiscal school year operating calendar dates and holiday schedule. The facility will be closed for the summer, last day of operation is June 10, 2022 to August 12, 2022 and scheduled date to re-open is on August 15, 2022.

Normal Days/Hours of operation are as follows for Two Separate Programs:

2-day program/Weekly: Tuesdays & Thursdays 9:00 AM to 2:00 PM

3-day program/Weekly: Monday, Wednesday & Friday 9:00AM-2:00PM.

The inspection consisted of reviews of the following domains: Food Service; Reporting Requirements


Physical Plant; Care and Supervision; Children Records; Staff Records; Staffing Ratio and Capacity and
Personal Rights. The inspection found the facility to be in compliance in these domains, except as noted on the LIC809D. Deficiencies were not cited this visit.
The licensee is asked to update the following documents, if applicable, and submit to licensing within 30 days:

3. Parent Handbook/ Program Curriculum/Admission policies and procedures/ fee schedule (only if changes have been made)

The following items have been posted and are updated where necessary:

· The facility is operating within the terms of the license on 05/25/2022
· Ratios were met and appropriate supervision was provided during this inspection. The facility has a rotating schedule for enrolled students, Mon/Wed/Fri schedule and T/Th schedule with no more than 60 students present per day.
· Classrooms are clean, free of hazards and equipped with age appropriate furniture and equipment in good condition
· No weapons stored at the facility on 05/25/2022.
· There are no accessible bodies of water present on 05/25/2022. All wading pools or similar product must be emptied immediately after use and stored in an upright position.
· Medications are stored where inaccessible to children in the director’s office and staff have access to the medication lock box in the event of an emergency. No children are currently taking medications on 05/25/2022.
· Hazards are stored where inaccessible to children which include: Disinfectants, cleaners and other items that are dangerous. Poisons and toxins are locked in the kitchen storage closet.
· All floors are clean and safe on 05/25/2022.
· Bathrooms were observed to be safe, sanitary and in operating condition on 05/25/2022
· Outdoor activity areas/play grounds are enclosed by appropriate fences, free of hazards and are supplied with age and size appropriate equipment in good condition on 05/25/2022.
· Food preparation area is clean, free of litter, rubbish and free of rodents and other vermin.
· Measures are taken to keep the facility free of flies, other insects and rodents. The licensee has an exterminator contracted to spray outside on a monthly basis.
· Food is stored appropriately and protected from contamination. LPA S. Brewer, consulted with Director Jane Osuna, regarding labeling of stored snacks when removed from the original packaging.
· Storage containers for solid waste, including moveable bins-have tight-fitting covers in good repair.
· Menus are posted at least one week in advance in a place visible by the child’s authorized representative, dated and kept on file for 30 days, and made available upon request. The licensee only provides snacks at the facility. LPA verified snack menu posted on 05/25/2022.
· Uncontaminated drinking water shall be readily available both indoors and out and provided by water bottles that are brought from home.

If a child forgets to bring their water bottle from home, the facility has water bottles to provide for each child. The outdoor water fountain with three drinking spouts is currently turned off due to COVID-19 preventative measures. LPA verified water bottles stored and a water dispenser with disposable drinking cups on 05/25/2022.
· The areas around or under high climbing equipment, swings, slides, and similar equipment shall be cushioned with material that absorbs a fall: Facility has a sand box, artificial turf areas and play area with wood chips to absorb falls.
· Sign in/Sign out record was reviewed and meets regulation requirements: verified on 05/25/2022
· A Staff member is present with current Pediatric CPR/First Aid which expires on 07/2023 and Director who renewed training online only 01/2022.
· Opening and closing staff member’s CPR/First Aid expires on 07/2023.
· Director completed Health and Safety Training on Spring 2004.
· Staff qualifications and files were reviewed and are NOT complete: Staff #6 Missing MMR, 7 staff failed to renew their Mandated reporter training, Director completed CPR/1ST Aid Training online.
· Staff have received on the job training for housekeeping, sanitation and universal health precautions
Licensee Director Jane Osuna, was informed the Department has inspection authority per Health and Safety Codes.

· Documentation of fire & earthquake drills to be conducted every six months: Last drill for Tue/Thu class 04/21/2022 at 10:00 AM and lasted 2 minutes; Mon/Wed/Fri class 04/20/2022 and lasted 2 minutes, evacuations to the parking area.
AB2370 – Lead Exposure, day care facilities, effective January 1, 2019 – The bill requires that all Child Care Centers, operating in a building constructed before January 1, 2010, shall have their drinking water tested for excessive lead levels, on or after January 1, 2020, but no later than January 1, 2023. Child Care Centers must thereafter test their drinking water every five years after the date of the initial test.

AB2960 – Child care and development services, online portal – Effective June 20, 2022 - This bill requires the State Superintendent of Public Instruction (SSPI), within the California Department of Education (CDE), to develop and post on CDE's website a comprehensive child care and development services online portal for families and providers by June 30, 2022.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. Facility Representative Jane Osuna, was reminded that all adults 18 and over living or working in the home, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPA S. Brewer, discussed the safe sleep regulations with Director Jane Osuna, and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. There are no infant children enrolled at the facility.
