On 08/19/2021 at 11:19am, Licensing Program Analyst (LPA) Destinee Hogue conducted a case management inspection to follow-up on an Unusual Incident Report (UIR) submitted by the facility to the Riverside Child Care Regional Office via fax on 08/03/2021. Director, Jami West has requested a temporary waiver to utilize alternative rooms at Calvary Presbyterian Church located at 4495 Magnolia Ave, Riverside, 92501, until repairs are completed in licensed Rooms A, B, and C. A fire inspection for temporary rooms 1-5 was conducted on 08/19/2021. During this inspection, LPA Hogue obtained a copy of "Notice of Fire & Safety Violation(s)" form, which was provided by City of Riverside, Fire Department-Prevention Division. Based on the documentation obtained during this inspection, it was noted by City of Riverside Fire Department that "no violations were noted at the time of fire department inspection." Temporary relocation waiver effective on August, 19, 2021 through February 19, 2022. During this inspection, LPAs toured the facility inside and outside, measurements were taken, and the following was observed and discussed with Director, Jami West.

On Tuesday, August 3, 2021 at 1:30am, the Director received a phone call that the school-age classroom "A" had caught on fire. Director immediately contacted parents via telephone and informed parents the classroom is closed and will temporarily relocate to another room at the property. Classrooms A, B, and C have not been in use by school-age children since, August 3, 2021.

During this inspection, LPA took measurements at Calvary Presbyterian Church rooms 1-5, and all other school-age activity areas.

LPA has determined that there is sufficient space to accommodate the requested capacity.

Preschool Outdoor Activity Area:
LPA has determined that there is sufficient space to accommodate the requested capacity.

Limiting factor for school-age capacity is bathroom fixtures. School-age capacity is limited to 37 children. Proof of completed fire inspection has been obtained for Rooms 1-5, however the Department has not received form STD850 from City of Riverside, Fire Department-Prevention Division.

LPA observed the following during this inspection:
· Hazards are stored where inaccessible to children which include: disinfectants, cleaning solutions and other items that are dangerous to children.
· Poisons and toxins are locked
· Medications are stored where inaccessible to children
· Classrooms are equipped with age appropriate furniture and equipment in good condition
· Outdoor activity areas are supplied with age and size appropriate equipment in good condition
· Playgrounds are enclosed by appropriate fences and free of hazards
· The surface of the outdoor activity space is free of hazards.
· All toilets, handwashing, and bathing facilities are in safe and operating condition, at this time.
· All floors were observed to be clean and free from hazards
· Food preparation area is clean and free of vermin
· Food is stored appropriately and protected from contamination
· All storage containers for solid waste, including moveable bins have tight-fitting covers that were on and in good repair, at this time.
· The areas around or under high climbing equipment, swings, slides, and similar equipment shall be cushioned with material that absorbs a fall.
· The Licensee shall ensure the facility is free of flies, other insects and rodents.
· Appropriate carbon monoxide detector is present and was tested by the Director during this inspection.
· Director understands the Child Care Center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors.

The following items are required prior to approval of temporary usage of the rooms 1-5:

1. Director/Licensee agrees to take measures to ensure the indoor and outdoor activity areas are free of flies, vermin, and other insects. Director stated she will be purchasing an "insect fogger," as a measure to ensure the indoor and outdoor activity areas are free of flies, vermin and other insects. Director agrees to schedule a pesticide appointment if issues with flies, vermin, and other insects continues to be an issue at the facility.


2. Outdoor play area shall be free from hazards and debris. Director agrees to have holes, glass, and rocks cleaned prior to use. During this inspection, maintenance began cleaning outdoor debris. Director stated maintenance will complete clean up work by the end of day (08/19/2021)

Once the above have been completed, with proof sent to licensing, the temporary waiver will be granted and waiver terms and conditions will be defined in finalized temporary waiver. Once waiver is provided to facility, facility is required to post temporary wavier in a visible location at the facility.

Failure to comply with the conditions of the waiver, may result in termination of the waiver. Directors agrees to provide the Department with updates regarding the completion and repairs of the licensed school-age Classrooms A, B, and C.

No deficiencies were cited during this inspection.

An exit interview was conducted, and a Notice of Site Visit was provided and must be posted for 30 days. The signature below acknowledges these documents were received on 08/10/2021 by Director, Jami West.
