Licensee has requested for capacity increase from 28 children to 60 children. Licensing Program Analyst (LPA) Kim Leung conducted a comprehensive inspection this date on 6/11/2021. A tour of the inside and outside of the facility was granted and the following was observed and/or noted:

School-Age Indoor Activity Areas
Room 24 and Room 25 are used for indoor activities. Facility is located on school ground and is exempt from indoor square footage requirements per Health and Safety Code 1596.806(a)(1) and (d) at this time.

School-age Bathroom Fixtures
There are 4 portable restrooms including one staff restroom, three children's restrooms and one employee's restroom. As stated by the facility director Justin Hickey, the employee's restroom will be used as the isolation restroom for ill child(ren) when needed once the increase has been approved. Each portable restroom has one toilet and one sink with running water. As stated by the director, the portable restrooms are cleaned and sanitized prior to children's arrival and throughout the day on a frequent basis. As stated by the director, all porta potties are emptied out once a week prior to children's arrival per the supplier's recommendations. There is an additional sink in each of the two activity rooms. A waiver is on file allowing the use of portable restrooms. As stated by the director, facility is obtaining authorization from the school to share the students' restrooms with the children care children.

School-Age Outdoor Activity Area:
The playground behind the activity rooms is exclusively used by the YMCA program for outdoor activities. Facility is located on school ground and is exempt from outdoor square footage requirements per Health and Safety Code 1596.806(b)(1) and (d) at this time.

A review of a sampling of the staff records and children's records were conducted as part of this evaluation.
· Updated forms are on file
· The following items have been posted and are updated where necessary:
- License
- Emergency Disaster Plan (LIC610) and Earthquake Preparedness Checklist (LIC9148)
- Parent’s Rights Poster (PUB393)
- Personal Rights (LIC613A)
- Child Car Seat Law
- Snack Menu
· The facility is operating with the limits as stated on the license.
· Ratios are being met during this inspection.
· Classrooms are adequately equipped with age and size appropriate furniture and equipment and free of hazards. The carbon-monoxide detector was tested by staff during inspection and it's found to be in working condition.
· There are no weapons present at the facility as stated by facility director Justin Hickey
· There are no accessible bodies of water present. All wading pools or similar product must be emptied immediately after use and stored in an upright position.
· Bottled drinking water will be provided to the children indoor and outdoor throughout the day. There are drinking faucets inside the activity rooms but those are not used during the COVID-19 pandemic.
· The entire playground is surrounded by chain-link fence of at least 4-foot tall
· There is an age appropriate play structure on the playground
An adequate amount of cushioning material (rubber filler) is in place under play equipment
· The trees provide shade. Director stated that a canopy is placed on the playground to provide additional shaded area each time prior to children entering the playground.
· No medications are stored for the children at this time as stated by staff
· Hazards are stored where inaccessible to children which include: Disinfectants, cleaning solutions and other items that are dangerous
· Poisons and toxins are locked
· All floors are clean and safe
· The portable restrooms were observed in operating condition. Currently, the portable restrooms are not temperature controlled. Director is obtaining authorization from the school to share the students' restrooms with

the children care children to prevent heat-related illnesses. COVID-19 guideline on maintaining the restrooms clean and sanitary after each use was reviewed with director and assistant director during this inspection.
· Children bring their own lunch. Hot running water is not available at this facility. Facility provides morning and afternoon snacks that require no cooking. Disposal utensils and plates are used. Food preparation area is clean, free of litter, rubbish and free of rodents and other vermin
· Food is stored appropriately and protected from contamination
· All storage containers for solid waste, including moveable bins have tight-fitting covers that are kept on, and in good repair
· Sign in/Sign out record was reviewed and meets regulation requirements
· A Staff member is present with current Pediatric CPR/First Aid which expires in October 2021 (Brittany Price)
· Opening and closing staff member’s CPR/First Aid expires on 9/21/2021 (Justin Hickey and Brittany Price)
· Director completed Health and Safety Training
· The Licensee was informed of their reporting requirements and is provided with the Regional Office’s Unusual Incident Reporting email mailbox: UnusualIncidentReportsDO09@dss.ca.gov, and UnusualIncidentReportsDO10@dss.ca.gov
· A review of children’s records was conducted and records were found to be complete during this inspection.
· Documentation of fire & earthquake drills was reviewed. The most recent earthquake and fire drill was conducted on 6/3/2021.
· A review of staff records on 6/11/2021 indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions.
· A review of staff records indicates that all staff present meet minimum qualifications for the position for which they were hired.
· The licensee was advised that the Notice of Site Visit must be posted at the entrance of the facility for a period of 30 days. If a serious violation is cited, a copy of the licensing report (LIC809 or LIC9099) must be posted for 30 days.
· Facility is not currently providing IMS Incidental Medical Services (IMS) policy was discussed during previous inspection. For IMS information see Evaluator Manual – Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided an updated Plan of Operation that includes IMS must be submitted to the Department. Updated child care regulatory requirements were provided to the facility during previous inspection on 3/2/2021.

The following items need to be completed prior to approval of capacity increase:


1. Submit written authorization from the school to share the students' restrooms with the children care children.

An exit interview was conducted and during the interview, the director Justin Hickey confirmed that there are no Registered Sex Offenders living in the facility and/or using the facility address for their mailing address.

A fire clearance for 60 children is on file. The application for capacity increase will be submitted for approval once the above listed have been completed with proof submitted to Community Care Licensing.

A NOTICE OF SITE VISIT WAS ISSUED AND LPA VERIFIED THAT IT WAS POSTED IN A PROMINENT LOCATION AT THE FACILITY BEFORE LEAVING. THE LICENSEE UNDERSTANDS THAT IT MUST REMAIN POSTED FOR THE NEXT 30 DAYS. This report must be available for review, upon request, for the next 3 years.
