25 | Licensing Program Analysts (LPAs), Joanne Domingo & Nasha King conducted a follow up Case Management Plan of Correction (POC) Inspection to verify the following corrections have been completed. The initial visit was conducted on 7/23/21. A capacity increase was requested on 6/18/21 from 30 children to 79 children and a fire clearance was approved and granted on 7/14/21. There were no children present during this visit.
1. Waiver for Classroom #5 (located in the Sanctuary) to use women's bathroom.
Per LPM, Kimberly Williams, a bathroom waiver for the classroom located in the Sanctuary is not needed as it is the only classroom and bathrooms located in the sanctuary and will not be used by any other persons.
2. Partition walls for Classroom #4 (located in the Fellowship Hall),
Partition walls have been placed in the Fellowship Hall to separate the classroom
3. Two Door Alarms for Classroom #4's Exit Doors.
Three contact door alarms were installed for the main entrance and two doors located in Classroom #4.
4. Rocks to be filled in the 4-tiered fountain and water level to be lowered.
River rocks were added to all four tiers and water level was reduced so as to not rise above the rocks.
5. Staff #4 to submit current Mandated Reporter Certificate. Staff #4's Mandated Reporter Certificate was received on 7/23/21.
The application for Capacity Increase will be submitted for approval with a maximum capacity of 79 children.
An exit interview was conducted, a Notice of Site Visit was posted, along with a copy of this report were provided to Licensee, Jennifer Jackson.
This report must be available for review, upon request, for the next 3 years. |
