At 9:00 a.m. on Friday, October 1st, 2021, Licensing Program Analyst (LPA) Karyn Guerra met with Director, Paula Martinez, for the purpose of an unannounced POC inspection regarding a citation from 07/28/2021. A risk assessment for COVID-19 was conducted prior to entry into the facility. At 9:08 a.m., LPA observed a census of 9 children supervised by 3 staff. The facility was previously cited for mandated reporter training. LPA observed updated mandated reporter training certificates for all staff at the facility. Mandated reporter training citation has been corrected per POC and has been cleared.
