On April 28, 2023, at approximately 9:20 AM Licensing Program Analyst (LPA) Josiah Gathing met with Director Kayla Gargalikis to conduct an unannounced case management inspection. During today's inspection there were 21 preschool children being supervised by 3 teachers. The purpose of today's inspection was to create a plan of correction following reports of lead exceedance in three of the facility outlets.

On March 3, 2023, the facility tested water samples for lead. Three water outlets sampled indicated an Action Level Exceedance (ALE). These outlets are marked F, H, and I on the facility sketch provided by the center. All three faucets are located near the entrance to the Toddler room. This room has not been in use since late 2021, and the faucets have not recently been used for drinking water. On March 27, 2023 the facility was notified that water levels were in exceedance of 5 parts per billion in the locations that were tested.

Signs have been placed on each outlet in exceedance stating that the outlets should not be used for drinking or food preparation. Director stated that the faucets will be replaced and re-tested for lead level exceedance. LPA received the lead testing map and other required documents from the Director during today's inspection.

A deficiency is cited on the following LIC809-D and the plan of correction was reviewed with the Director, Kayla Gargalikis. A copy of this report and appeal rights were provided to the Director.

Director will submit a plan to the LPA demonstrating that use of the outlets in question will permanently cease, or the outlets will be replaced. Center will also notify parents about what has been done to date (e.g. signs, plans to replace outlets, etc).

Based on record review the facility did not comply with the above regulation as the lead action level was exceeded in three water outlets, which poses Health, Safety, or Personal Rights risk to persons in care.
