On 10/7/2022, at approximately 8:45 AM, Licensing Program Analyst (LPA) Alize Tillery and Licensing Program Manager (LPM) Seychelle De Luca, conducted an unannouced visit to further investigate and conclude the complaint investigation for the allegations listed about. During today's vist, LPA and LPM observed 9 preschool children, in the purple room, supervised by 2 staff.

One allegation is that staff are not changing day care children's diapers in a timely manner. Staff interviews revealed that staff #1 and staff #2 failed to attend to children with wet diapers, resulting in serious diaper rashes and parents complaining that their children are being left in the wet diapers. Interview with staff memeber also revealed that staff #1 and staff #2, left a child in wet clothing during nap time, after child had engaged in outdoor water play. Staff interview revealed that staff #1 and #2 threw the child's wet clothing in the trash instead of placing the clothing with their belongings. This is a violation to children's personal rights. Staff members stated that staff #1 and staff #2 are no longer employed with the facility and that current staff now ensure that children's diaper and clothing are checked regularly. LPA also observed that staff members are documenting each time a child has their diaper changed.

The second allegation is that there is a lack of supervision of children in care. During staff interviews, LPA and LPM were informed of two separate incidents that involved a lack of supervision of two separate children. Staff members revealed that on one incident, Child #1 left the classroom from the outdoor play area, entered the building on their own and got out the front/main entrance door of the building. Staff # 3 stated that while on break in the office area, Staff #3 was able to chase after Child #1 as Child #1 was exiting the main entrance of the facility. Staff #3 warned staff #1 and staff #2 that Child #1 is a runner, as it was child's first day back in care. This incident happened due to a lack of supervision and this poses an immediate risk to children in care.

Staff members revealed that on a separate occasion, Child #2 was unaccounted for and forgotten on the outdoor preschool play area for approximately 3 minutes. Staff members stated that Staff #4, who was on the outdoor infant play area, observed the child to be alone. Staff members stated that Child #2 was not counted for during their classroom transition back inside. Staff members stated that there was a miscommunication amongst staff members, and that staff now ensure to count by child's face before and after transition periods. This incident occurred due to a lack of supervision; this poses and immediate risk to children in care.

Director acknowledges, that for TYPE A DEFICIENCIES ONLY upon receipt, Director shall post LIC 9099-D with Type A deficiency for 30 days and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months. LIC9224 must be signed by parents/guardians and kept with the children's forms as a receipt whenever any Type A documents are provided by the Director. LIC9224 and Appeal Rights were provided. An exit interview was conducted and a Notice of Site Visit posted.

On 10/7/2022, at approximately 8:45 AM, Licensing Program Analyst (LPA) Alize Tillery and Licensing Program Manager (LPM) Seychelle De Luca, conducted an unannouced visit to further investigate and conclude the complaint investigation for the allegations listed about. During today's vist, LPA and LPM observed 9 preschool children, in the purple room, supervised by 2 staff.

One allegation is that the day care does not have proper ventilation. LPA is aware that the building that the facility is in is undergoing reconstruction and staff do not have access to control the main heating or air conditioning system. As of Spring 2022, the facility has been using swamp coolers throught the facility to maintain a comfortable temperature in the rooms. Director stated that maintenance visits the facility daily to ensure that a comfortable temperature is kept and maintenance will adjust the swamp cooler if need be. During LPA visit on 9/1/2022 and today, LPA did not observe the temperature of the rooms to be uncomfortable. Based on LPA and LPM observations, and information received during staff interviews, the allegation, day care doesnt have proper ventrilation, is unsubstantiated.

The second allegation is that "Drinking water filters have mold in them". During LPA visit on 9/1/2022, and today, LPA did not observe any mold in the water filters/britas. During staff interviews, LPA learned that staff switch the filters monthly and clean the britas daily. Based on observations and information received during staff interviews, the allegation, Drinking water filters have mold in them, is unsubstantiated.

LPA reviewed and provided the report and appeal rights to Director. LPA also provided a notice of site visit and Director acknowledges it is to remain posted for 30 days.

Staff interviews revealed on a separate ocassion, Child #2 was forgotten on the outdoor area for approx. 3 min. before teacher returned for Child #2.
