25 | At 9:40 a.m. on Friday, June 30th, 2023, Licensing Program Analysts (LPAs) Karyn Guerra and Pa Dao Vang met with Representative, Martha Garcia, for the purpose of an unannounced case management - incident inspection regarding a self reported incident that occurred on 6/13/2023. LPAs observed a census of 10 preschool children supervised by 1 staff, and 6 toddler children supervised by 2 staff. An unusual incident report form was received by the department on Wednesday, June 28th, 2023. It was learned that a child (C1) from the toddler classroom was left alone unsupervised on the outdoor play yard for a period of 5-10 minutes while the rest of the classroom had transitioned inside. It was stated in interviews that no staff or children were in the infant classroom, whose windows face the outdoor play yard. It was stated that the child was later found near the outside door by a staff who was conducting closing duties for the day, and taking out the garbage. The child was stated to be in the main preschool play yard portion of the outdoor space. This poses an immediate risk to the health and safety of children in care. Civil penalties were assessed. The facility provided a name to face sheet that is now used to document ratios of children during transition.
Title 22 deficiencies are cited on the subsequent pages of this report. Licensee acknowledges, that FOR TYPE A DEFICIENCIES ONLY upon receipt, licensee shall post LIC 809D with Type A deficiencies for 30 days and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months. LIC 9224 must be signed by parents/guardians and kept with the children's forms as a receipt whenever any Type A documents are provided by the licensee. LIC 9224 and Appeal Rights were provided. Head Teacher's signature on this report acknowledges receipt of these rights. This report was reviewed with the Representative, Martha Garcia. An exit interview was conducted. A Notice of Site Visit was provided and shall remain posted for a period of 30 days. |
