25 | Licensing Program Analysts (LPAs) Seychelle De Luca and Marea Behvand met with Director Tonya Vorobets and Administrator Ryan Suarez for an unannounced case management inspection to discuss the Unusual Incident Report (UIR) that the facility called in and submitted on 7/2/2019 and which occurred on 7/1/2019. Upon arrival LPAs observed 17 napping preschool children and three staff in the Ferrets classroom, 19 napping preschool children and three staff in Dolphins classroom, and 18 napping preschool children and two staff in the Turtles classroom. LPAs interviewed staff, reviewed video footage, and toured the facility indoors.
It was reported that on 7/1/2019, Child #1 exited the Ferrets classroom and the front lobby to find Child #1's parent, who was in the parking lot. This occurred without Director or Staff #1 seeing Child #1 leave the classroom. Child #1's parent had not driven away yet and brought Child #1 back to the Ferrets classroom.
During today's inspection, LPAs reviewed video footage. LPAs observed Child #1 and parent entered the Ferrets classroom at 7:36 AM. Child #1's parent returned to the car at 7:40 AM. Approximately 30 seconds later, Child #1 ran out of the classroom and stood in the open doorway between the front lobby and the parking lot. Child #1's parent got out of the car and brought Child #1 back inside at 7:41 AM.

Director acknowledges, that FOR TYPE A DEFICIENCIES ONLY upon receipt, Director shall post LIC 809D with Type A deficiency for 30 days and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months. LIC 9224 must be signed by parents/guardians and kept with the children's forms as a receipt whenever any Type A documents are provided by the licensee. LIC 9224 and Appeal Rights were provided. |
