At 9:00 a.m. on Tuesday, September 3rd, 2024, Licensing Program Analyst (LPA) Pa Dao Vang met with Licensee, Nancy J. Ennisfor, the purpose of an unannounced, annual required inspection. LPA observed a census of 53 children supervised by 6 staff. Facility hours of operation are from 6:45 a.m. - 5:30 p.m., Mondays thru Fridays. Center provides breakfast, a.m. snack, lunch, and p.m. snack for the children.

Licensee was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPA conducted a health and safety inspection of the classrooms, restrooms, and food service area. LPA observed the following documents are posted: License, Emergency Disaster Plan, Personal Rights, Parents' Rights Poster, Earthquake Preparedness, Child Passenger Restraint Poster, Parent’s Notification, LIC613A Person Rights, Daily Activity Schedule, and menus. Licensee stated that there is no smoking, no firearms nor poisons on the premises. LPA observed the last fire drill log on 8/2/2024.

(1) Commencing September 1, 2016, a person shall not be employed or volunteer at a day care center if he or she has not been immunized against influenza, pertussis, and measles. Each employee and volunteer shall receive an influenza vaccination between August 1 and December 1 of each year.

Based on observation, interviews, and record reviews, the licensee did not comply with the section cited above in 3 out of 6 staff files missing immunizations (MMR, TDAP, and/or flu), which posed a potential health, safety or personal rights risk to persons in care.

LPA conducted a health and safety inspection of the outdoor space. The play structure is free of loose or sharp parts with proper cushioning under it. Outdoor shade was provided by canopy, building, and trees. Water is accessible via water bottles and a water fountain. The water bottles are labeled with children’s names and transitioned with children to different environments.

LPA reviewed staff and children's records. LPA observed current employee rights, and documentation of the educational background, training, and/or experience. There is at least 1 staff with current CRP/First Aid certification in the facility. LPA observed 3 out of 6 staff missing current immunization records in files.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For childcare center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP). LPA obtained a copy of the lead testing completed in 3/20/2023 with a Action Level Exceedance of 6.46ppd in the water fountain outlet previously located in Room 3. Licensee stated the water fountain was immediately removed and capped off since last year.



Facility Representative was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain childcare by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California.

A deficiency is being cited based on LPA’s observation, interviews conducted, and record reviews in accordance with the California Code of Regulations, Title 22, see LIC809-D page. An exit interview was conducted with Licensee, Nancy J. Ennisfor. A copy of the appeal rights and a notice of site visit were also provided to Licensee. Notice of site visit shall remain posted for a period of 30 consecutive days.
