Licensing Program Analyst (LPA) Pa Dao Vang met Licensee Nancy J. Ennis to conduct a Plan of Correction (POC) inspection on 9/3/2024 at around 9:00 PM. A Type b deficiency was cited on 3/2/2023 for the Action Level Exceedance of 6.46ppb for the outlet water fountain located in Room 3. Licensee stated the water fountain was immediately removed and capped off since last year.

On today's date, LPA observed 53 children supervised by 6 staff. POC for lead exceedance is cleared as of 9/3/2024.

This report was reviewed with Licensee Nancy J. Ennis. A copy of this report, appeal rights, and a notice of site visit was provided to be posted for 30 consecutive days.
