Due to COVID-19 pandemic, Licensing Program Analyst (LPA) Mai Lor conducted a case management tele-inspection via FaceTime with Director Leah Sarmiento. The purpose of this case management is to follow-up on a unusual incident report (UIR) received in the Sacramento Regional Office on 10/14/2020. Census included 112 preschool children.

LPA Lor discussed the usual incident with the director. LPA Lor advised and reminded the director the importance of care and supervision at all times. Director stated administrative action had been taken and care and supervision policy was reviewed with the employee.

No Title 22 Regulations cited. Exit interview conducted. In lieu of director’s signature, LPA Lor is emailing the director a copy of this report and notice of site visit with a read receipt request.
