25 | On 8/14/2024 at 9:00a.m., Licensing Program Analysts (LPA) Pa Dao Vang met with Director, Stephanie Levenhagen, for a Case Management inspection regarding an Unusual Incident Report called in to our regional office on 8/1/2024. Upon arrival, LPA observed 25 preschoolers supervised by 3 staff.
It was reported on 7/25/2024 approximately at 11:45a.m., C1 was fell from the stairs in the playhouse loft area located in the Blue classroom. C1 broke the radius and ulna bones on the right arm. Parent were called immediately and picked up C1 to the hospital.
During today's inspection LPA toured the facility, made observations, conducted interviews, and inspected the classroom. LPA inspected the loft area where the incident took place. Through the staff and children’s interviews, LPA learned there was one Teacher cleaning the in the sink area across the classroom from the loft area. According to the Teacher, she didn’t observe the incident because she was cleaning the paint brushes in the sink, but heard “Bump-bump and crying.” She then attended to C1 sitting the bottom of the stairs with the right arm broken. Throughout the course of the investigation, there are a supervision violation in which the teacher does not have visual supervise in loft area during the incident.
According to the Director, it was an accident. She beliefs that C1 fell because of the accident and not supervision.
Based on LPA observations and interviews conducted, a deficiency is being cited on page LIC809-D in accordance with the California Code of Regulations, Title 22.
