Licensing Program Analyst (LPA) Kelly Ferrara met with Director Lauri Lish for the purpose of an unannounced random annual inspection. LPA toured the facility including all activity and classroom spaces, restrooms, food service and outdoor play areas. Appropriate ratio and care and supervision were observed during today's inspection. Census included 13 school age children being supervised by two staff. Director was reminded never to exceed the conditions, limitations and capacity specified on the license. Facility hours of operation are Monday through Friday from 6:15 AM- 615 PM.

Classrooms all appeared clean including the carpets and floor. Playground equipment and surfaces are free of loose or sharp parts. The areas around or under the climbing equipment are cushioned with enough material to absorb a fall and there is adequate shade. Uncontaminated drinking water was readily available to children both indoors and outdoors. Bathrooms were clean and all sinks and toilets were in operating condition. There are no firearms or bodies of water on the premises, and the facility has at least one functioning carbon monoxide detector. Trash cans containing solid waste had lids. Menus were posted for lunch and AM and PM snack and other required licensing documentation was posted for parental review. LPA observed a computer where the parents are signing their children in and out of the center electronically.

A sample of children and staff's files were reviewed. All children's files contained the required documentation. Three out of eight staff files reviewed did not have proof of immunization to MMR and Pertussis.At least one staff member present today has current Pediatric CPR and First Aid certification that expires in June 2021. LPA observed a current children's roster and fire drill log with the last drill conducted in July 2019.

All staff currently employed with the facility has a criminal record clearance and are associated to the preschool and school age license. LPA reviewed the Department's inspection authority and discussed with Director any changes that may occur regarding the director or an employee acting in the director's absence must be reported to department within 10 working days.

Incidental Medical Services (IMS) policy was discussed although there are no children currently enrolled with medication. The facility has a first aid box with ample supplies. For additional IMS information, director was advised to see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226.The following information regarding ADA was provided: US Department of Justice (USDOJ) toll-free ADA Information Line at (800) 514-0301 (voice)/ (800) 514-0383 (TTY) and link to publication: Commonly Asked Questions about Child Care Centers and the ADA, available at: http://www.ada.gov/childqanda.htm.

This facility evaluation report was reviewed and discussed with the Director. Director was encouraged to the visit the departments website at WWW.CCLD.CA.GOV for information regarding child care updates, forms, regulations and legislation pertaining to child care centers. LPA gave the Director a hand out on the Effects of Lead Exposure and a hand out on Active Supervision. A notice of site visit was provided and should remain posted for a period of 30 days for parental review.

One Type B deficiency was cited based on today's inspection.

Employees or volunteers at day care center; immunization requirements; records; exemptions (a) (1) Commencing September 1, 2016, a person shall not be employed or volunteer at a day care center if he or she has not been immunized against influenza, pertussis, and measles. Each employee and

volunteer shall receive an influenza vaccination between August 1 and December 1 of each year. This requirement was not met as evidenced by: LPA conducted staff file review and observed three out of eight files did not have proof of immunizations. This poses a potential health and safety risk to children in care.
