On 7/20/2021 at 9:00am, Licensing Program Analyst (LPA) Jan Hoshida met with Assistant Director Betty Candy for a case management inspection regarding the unusual incident report (UIR) that the program submitted on 7/14/2021 and which the incident occurred on 7/13/2021. LPA 14 children with two staff present.

LPA conducted interviews, obtained documents, and observed care and supervision of children in care.

No deficiencies were cited in today’s visit.

Exit interview and Notice of Site Visit was posted.
