13 | On March 24th, 2021, Licensing Program Analyst (LPA) Jeremey McClain conducted a complaint inspection via Facetime Video with Dawna Allred. During the inspection, LPA observed 25 school age children supervised by two teachers. It was alleged that a staff member spoke inappropriately to and around children in care. During the investigation, LPA conducted interviews with staff and children, as well as reviewed documents provided by the Director. Evidence was conclusive that Staff 1 (S1), used inappropriate language towards a child (C1) in care, and also used inappropriate language while in the presence of children on another occassion. The preponderance of evidence standard has been met, therefore the allegation is determined to be substantiated. Title 22 deficiencies are cited on the subsequent page of this report. If not corrected, these violations pose and immediate risk to the health and safety of children in care. Director acknowledged that upon receipt of a TYPE A DEFICIENCY, an LIC 9099-D shall be posted for 30 days. Director also acknowledged that they must provide copies of this licensing report to parents/guardians of children in care and to parents/guardians of children newly enrolled at the facility during the next 12 months. LPA provided an LIC 9224, which must be signed by parents/guardians and kept with the children's files. Appeal Rights were provided. An exit interview was conducted, and a Notice of Site Visit posted. |
