25 | At 10:00 a.m. on Wednesday, December 23rd 2020, Licensing Program Analyst (LPA) Karyn Guerra conducted a tele-inspection and met with Director, Dawna Allred, for the purpose of a case management inspection regarding an unusual incident that occurred at the facility on December 15th, 2020. Director self-reported the incident to the department on Wednesday, December 16th, 2020. Census consisted of 4 infant children supervised by 2 staff, and 11 toddler children supervised by 4 staff. During today's inspection, LPA conducted interviews. LPA will follow up at a later time to conduct additional interviews due to staff absences.
No Title 22 Deficiencies were cited during inspection. LPA reviewed report with Director. A copy of the report and Notice of Site Visit will be provided electronically via email. Acknowledgement of delivery will be documented in lieu of signature. Notice of Site Visit must remain posted at facility for 30 days. |
