At 1:00 p.m. on Monday, November 15th, 2021, Licensing Program Analyst (LPA) Karyn Guerra met with Director, Dawna Allred, for the purpose of a complaint inspection regarding the above allegations. During today's inspection, LPA conducted interviews, made observations, and delivered findings. It was alleged that there was a lack of supervision resulting in inappropriate interactions between day care children while in care. There was a concern of inappropriate behavior from a child that was not addressed by the facility. Staff interviewed state that some children may be shadowed or redirected as necessary for behavioral needs that require extra supervision. LPA was unable to determine if there was a lack of supervision. It was also alleged that day care children are not allowed to nap while in care. There was a concern of children being woken excessively for diaper changes. Staff stated that diapers will be changed during nap time when necessary. Parents did not have a concern regarding napping practices or diaper

changes. It was also stated that some children in the classroom may cause noise that could wake others. Observations during nap time did not reveal any concerns. It was also alleged that staff falsify incident report information. The facility provides reports to parents and guardians for incidents and accidents that occur at the facility. There was concern that the facility did not reference that a child was pushed by another child in an incident report and instead fell. LPA reviewed report at the facility which referenced that child was pushed by another child. Staff interviewed denied falsifying any information. The allegations are unsubstantiated. Although the alleged violations may have occurred or are valid, the preponderance of evidence standard was not met to fully prove or disprove that the alleged violations did or did not occur, therefore they are unsubstantiated. An exit interview was conducted and a notice of site visit provided. Notice of site visit shall remain posted for a period of 30 days for parental review.
