Licensing Program Analysts (LPAs) Mai Lor and Christopher Bello met with Site Supervisor Melanie Lor for the purpose of an announced annual random inspection. The program operates in Room #2. Upon arrival, LPAs observed 14 children and two staff. LPAs toured the building including all activity and classroom spaces, restrooms, food service and outdoor play areas. Appropriate ratio, care and supervision were observed during the visit. Designee was reminded never to exceed the conditions, limitations and capacity specified on the license. The pre-school hours of operation are Monday through Thursday from 8:00am to 2:30pm and on Friday from 8:00am to 11:00am. The pre-school operates according to Sacramento City Unified School District (SCUSD) schedule.

LPAs observed that medications, toxic and hazardous items are all stored appropriately and inaccessible to children. Furniture and equipment are in good condition. Playground equipment and surfaces are free of loose or sharp parts. The areas around or under climbing equipment are cushioned with bark to absorb the fall. Toileting facilities are in safe and sanitary condition. The facility has a second bathroom that is on the school site. The facility provides breakfast, lunch and snacks. Food is provided through the school cafeteria. The food preparation space is free of litter. All food was protected against contamination. Trash cans containing solid waste had lids. Drinking water was readily available to children both indoors and outdoors. Children take naps. Menus were posted and sign in/out sheets consisted of full legal signatures. A random sample of children and staff files were reviewed. Five children records contained emergency information and consent for emergency medical treatment. At least one staff member present today has current Pediatric CPR and First Aid certification. Staff records contained all of the required documentation. A current disaster log and children roster is being maintained.

All staff currently employed with the facility have a criminal record clearance through the SCUSD. There are no firearms or bodies of water on the premises. LPAs did not observe a carbon monoxide detector.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. The following information regarding ADA was provided: US Department of Justice (USDOJ) toll-free ADA Information Line at (800) 514-0301 (voice)/ (800) 514-0383 (TTY) and link to publication: Commonly Asked Questions about Child Care Centers and the ADA, available at: http://www.ada.gov/childqanda.htm.

LPA Lor obtained copies of site supervisor's qualifications and experiences documentation. Lead brochure was reviewed with the site supervisor.

During today's inspection, the facility was cited for Type A deficiency. Licensee acknowledges, that FOR TYPE A DEFICIENCIES ONLY upon receipt, licensee shall post LIC 809D with Type A deficiencies for 30 days and provide copies of this licensing report to parents/guardians of children in care at the facility and to parents/guardians of children newly enrolled at the facility during the next 12 months. LIC 9224 must be signed by parents/guardians and kept with the children's forms as a receipt whenever any Type A documents are provided by the licensee. LIC 9224 and Appeal Rights were provided.



Exit interview conducted and this facility evaluation report was reviewed and discussed with the site supervisor. Site supervisor was encouraged to the visit the departments website at WWW.CCLD.CA.GOV for information regarding child care updates, forms, regulations and legislation pertaining to child care centers.
