Licensing Program Analyst (LPA) Gagandeep Singh met with facility representative, Tiffany Alvarez, for an inspection of plan of correction. Purpose of the inspection was explained.

During the inspection, LPA reviewed the staff files. During the record review, LPA observed all staff members have valid completion certificates for Mandated reporter training. This cleared the previously issued citation.

No deficiencies are observed today. Copy of this report is reviewed and provided to the facility representative. Notice of site visit is posted and shall remain posted for next 30 days.
